STATEMENT OF WORK (SOW)
1.0 TITLE
HVAC Services Contract
2.0 BACKGROUND
The Indian Health Service, Mashpee Wampanoag Health Service Unit is in need of an HVAC maintenance contractor to provide preventative maintenance services for our HVAC equipment.
3.0 OBJECTIVE
The Mashpee Wampanoag Health Service Unit would like to contract an HVAC maintenance contractor to provide preventative maintenance services.
4.0 SCOPE
The contractor shall supply all labor, tools, equipment, materials, transportation, management and supervision, and services necessary for heating unit and air conditioning unit performance through a service/preventative maintenance agreement. Performance standards must meet the government and state requirements for Massachusetts.
The services shall include:
• Routine preventive maintenance in accordance with manufacturer’s specifications for all major equipment systems installed at the facility, including repair parts and labor for the equipment.
• Emergency repair services of HVAC equipment and controls.
• Replacement and installation of HVAC components as required.
• Comprehensive system diagnostics to ensure system is operating properly.
• Detailed documentation and reporting of all services performed.
The Contractor shall keep informed of department process changes through communication with the Mashpee Service Unit.

5.0 CONTRACT REQUIREMENTS/PERSONNEL QUALIFICATIONS
The Contractor shall be currently licensed as an HVAC technician in Massachusetts.
5.1 Preventive Maintenance
The contractor shall:
• Provide and implement a preventive maintenance schedule based on manufacturers' recommendations and industry best practices for all major HVAC equipment systems installed at the facility.
• Inspect, clean, and replace air filters, belts, and other consumables.
• Lubricate all moving parts.
• Inspect and calibrate thermostats and controls.
• Test and verify proper operation of all HVAC components, including compressors, fans, and motors.
• Clean coils, condensate drains, and pans.
• Check refrigerant levels and inspect for leaks.
5.2 Emergency Repairs
The contractor shall:
• Provide on-call support for emergency repairs.
• Respond to emergency calls within hours.
• Conduct root cause analysis and resolve system malfunctions promptly.
5.3 Repairs and Component Replacement
• Notify the Government Representative of any required component replacements.
• Provide cost estimates for repairs not covered in routine maintenance.
• Install and test new components to ensure compatibility and functionality.
5.4 Reporting and Documentation
• Submit a detailed maintenance report after each service visit, including inspection results, completed tasks, and recommendations for further action.
• Maintain an up-to-date log of all services provided, including emergency repairs and replacements.
5.5 Compliance and Safety
• Ensure compliance with all applicable federal, state, and local regulations, including EPA standards for refrigerant management.
• Follow all safety protocols and ensure the safety of personnel, equipment, and property.
6.0 TYPE OF ORDER
Firmed Fixed Price
7.0 PERIOD OF PERFORMANCE
The period of performance shall be for an eleven-month base period with four, 12-month option periods.
Base Period: 02/01/2025 or Date of Award -12/31/2025 Option Period One: 01/01/2026 - 12/31/2026 Option Period Two: 01/01/2027 - 12/31/2027 Option Period Three: 01/01/2028 - 12/31/2028 Option Period Four: 01/01/2029 - 12/31/2029
8.0 PLACE OF PERFORMANCE
Place of performance will be the Mashpee Service Unit- 483 B Great Neck Rd South, Mashpee, MA 02649
9.0 REPORT(S)/DELIVERABLES AND DELIVERY SCHEDULE
The Contractor shall submit the following items within the required time frames to the COR/Technical POC:
The contractor shall provide the following deliverables:
1. Preventive Maintenance Schedule – A detailed schedule for routine maintenance.
2. Service Reports – A comprehensive report for each maintenance and repair visit, submitted within 48 hours of service completion.
3. Emergency Response Log – A log of all emergency calls, actions taken, and outcomes.
4. Inspection and Testing Records – Records of inspections, tests, and calibrations, updated quarterly.
5. Replacement Cost Proposals – Itemized cost proposals for recommended replacements or major repairs, submitted as needed.
10.0 PAYMENT
The Contractor shall submit invoice(s) through the Invoice Processing Platform (IPP) system after services have been received.
