Statement of Work National Centers for Animal Health Building MB-401 Floor Replacement

General Requirements

Scope of Work:

A. Contractor Scope of Work:

· Contractor shall remove all of the existing 4" vinyl wall base and carpeted floor in highlighted areas of the included drawing, any associated pad, any and all remaining adhesive down to bare/clean floor to prepare for installation of new vinyl composite floor tiles, new carpet tiles, new 4" vinyl wall base, and new transitions.

· All demolished materials shall be removed from the site and disposed of in an appropriate location by the Contractor.

· New floor tiles shall be vinyl composite tile by Armstrong. Model shall be 12" x 12" Standard Excelon, Imperial Texture, model 51904 (Contractor shall provide physical sample to the Government for verification prior to purchasing).

· New carpet shall be carpet tile by Interface. Model shall be 19.69" (50 cm) x 19.69" (50 cm) Jakarta II (Product Number 1617102500), Coconut Color (Color 002259). Contractor shall provide physical sample to the Government for verification prior to purchasing.

· Contractor shall prepare the floor for new carpet and vinyl composite floor tiles as per manufacturer recommendations.

· Contractor shall install new vinyl flooring in area marked on the drawing and shall install new 4" vinyl wall base manufactured by Roppe or approved equal (pewter color).

· Contractor shall install new carpet tile flooring in area marked on the drawing and shall install new 4" vinyl wall base manufactured by Roppe or approved equal (pewter color).

· Contractor shall install transitions in all locations in which there is a new change in flooring material.

B. Project Timeline:

Contractor shall be given 120 days from the "Notice to Proceed" to complete the project.

Work Restrictions & Conditions

A. Site Security Access:

1. Access to the site is controlled by a security fence and visitor/contractor check-in/check-out system. All contractor/subcontractor employees shall sign-in and out at the main entrance gate and wear a contractor/visitor identification badge at all times while on the site. The ID badges will be turned in at the end of the workday. A current government issued identification with a picture ID is required for entry.

2. This project site is located in the limited access zone. Contractor shall provide escorting for all workers except for any work in laboratories and shall use USDA workers for escorting in laboratories.

3. Contractor vehicles will be allowed access adjacent to the work; contractor employee's personal vehicles shall be parked in the facility parking lots.

4. The Contractor shall use only the service roads and access routes approved by the Contracting Officer's Representative.

5. Work shall be confined to the areas identified on the drawings. Unless accompanied by a NCAH/USDA employee, the Contractor, subcontractors, and their employees shall not enter buildings or facilities not specifically part of this project. Reasons included, but are not limited to, disease control and health requirements.

6. Additional security requirements may be imposed when the Federal Government raises the Homeland Security Alert Level. Additional directions will be provided if the situation occurs.

7. Failure to comply with access requirements shall be adequate grounds for removal of the Contractor/subcontractor employees from the site and barring further site access to individuals.

B. Work Hours: Regular work hours for the site are 8:00 am to 4:30 pm, Monday through Friday, excluding Government holidays.

C. Work Outside Regular Work Hours: Work scheduled outside regular work hours including late hours, Saturdays, Sundays, and Government holidays requires Contracting Officer's Representative (COR) approval in advance. Make application to the COR at least 5 calendar days prior to such work to allow arrangements to be made by the Government for access to the work area and inspection & monitoring arrangements. Requests shall include the exact dates, hours, location, and purpose of the exception. If approved, the COR will coordinate access with the site security office.

D. Construct Work in stages to accommodate use of premises and limited space to store furniture during the service period. The work should be conducted in a minimum of four phases to allow for furniture to only be removed from one half of the offices (carpet area) at one time or from one half of the corridor/breakroom (vinyl tile) area at one time. The Government will be responsible for moving furniture . Coordinate work schedule and operations with Contracting Officer's Representative (COR).

Utilities The existing USDA utilities at the site may be used by the Contractor for completion of the contract work at no cost to the Contractor. The Contractor shall be responsible for making and removing any temporary utility connections.

· Telephones: The Contractor shall equip all his foremen and work leaders with cell phones as necessary to coordinate the work effort. The Government will not provide any phone service to the project.

· Construction Trailer: There should be no need for a construction trailer at the jobsite. Contractor may store equipment tools, materials onsite as needed for the project in a Contractor provided, locked job-box.

· Toilet Facilities: A temporary toilet facility shall be provided by the Government.

Submittal & Drawings Procedures

· Documents will be submitted in PDF, scanned or created in documents that can be easily read when printed on 8-1/2 x 11 or 11 x 17 sheets.

· Submit manufacturer's details and specifications for devices and materials used in Section 1.0.

Close-Out Submittals

· Contractor shall provide final technical data to COR for review.

· Contractor shall coordinate progress inspections with the COR and before final payment shall request a final inspection. The COR will complete a punch-list and/or document to the Contracting Officer that the work is complete in accordance with the project requirements before final payment is made.

Warranty Documentation

· Warranty List: Contractor shall provide a warranty summary sheet in spreadsheet format with all the warranties listed by section, description, supplier, warranty contact, start date, end date and provide electronic copy to the COR and place printed copy in the front of the warranty binder.

· Warranty Copies in Binder: Contractor shall provide the COR one signed & dated copy of all warranties for products and equipment installed on the project. An electronic (scanned) copy of the warranty binder in a PDF file shall be submitted with the final close-out documentation.

· Operation & Maintenance Manuals: Contractor shall provide a printed copy of all Operation & Maintenance documentation provided with the equipment and systems installed in the project and place the manuals in an organized binder. The contractor shall also provide a PDF version of the Manuals to be submitted with the close-out documentation.

· A one-year warranty shall be provided for faulty workmanship and material. The warranty shall start at the completion of the project.

· The contractor shall provide technical specifications and product information along with the cost proposal.
