About this file
This document is the Standard Illinois Certifications form issued by the State of Illinois Chief Procurement Office for General Services, establishing mandatory compliance requirements for all vendors, contractors, and subcontractors engaging in state contracts. The certifications apply to the entire contract term and any renewal periods and are material requirements and conditions of the contract. Vendors must maintain continuous compliance throughout the contract period and report any non-compliance. The certifications cover multiple statutory and regulatory areas including entity status, educational loan defaults, early retirement incentive eligibility, legal entity authorization, business registration requirements, collective bargaining obligations, criminal convictions, felony convictions under securities law, contract prohibitions, debt delinquency status, tax compliance, environmental violations, anti-bribery provisions, revolving door provisions, lobbying registration, anti-collusion reporting, domestic product sourcing, child labor prohibitions, lead poisoning prevention, Executive Order compliance, information technology accessibility, political contribution restrictions, domestic products procurement preferences, and forced labor prohibitions.
Vendors certify compliance with numerous Illinois and federal statutory requirements including provisions related to educational loans (5 ILCS 385/3), pension code requirements (30 ILCS 105/15a and related sections), procurement code provisions (30 ILCS 500 series), environmental protection (30 ILCS 500/50-14), bribery and anti-competitive practices (30 ILCS 500/50-5, 50-10, 50-25, 50-30, 50-38, 50-40, 50-45, 50-50), tax compliance (30 ILCS 500/50-12), drug-free workplace requirements (30 ILCS 580), export administration restrictions (30 ILCS 582), human rights and employment practices (775 ILCS 5/2-105 and 25/2), and forced labor prohibitions (30 ILCS 583-584). False certifications subject vendors to contract voidance, state declaration of contract void, suspension, debarment, denial of payment, civil fines, and criminal penalties under the Illinois False Claims Act (30 ILCS 500/50-2). Subcontractors must be included in all certifications, and contractors must report suspected collusion, anti-competitive practices, or other violations to the Illinois Attorney General and Chief Procurement Officer.
