PAST PERFORMANCE QUESTIONNAIRE
Instructions to Offerors (Section A):
Section A shall be completed by the Offeror (the firm requesting the reference) prior to issuing the questionnaire. The Offeror shall send the questionnaires for all recent and relevant projects listed in response to the RFP Section L, Factor III Past Performance, to individuals identified as being able to provide a reference. For Government contracts, send the questionnaire to the Government Contracting Officer, Contracting Officer’s Representative, or Technical Representative. For commercial contracts, send the questionnaire to those personnel with similar duties as those for Government contracts. It is the responsibility of the Offeror to encourage references to submit a questionnaire.
All projects submitted must be determined as recent, meaning that they are in progress or have been completed in the past five (5) years based on the proposal submission date for this RFP. If the offeror intends to use this reference for more than one submission (ex: If proposing on Weather IDIQ on prime and subcontractor teams), it must be clearly delineated in Section A of the questionnaire. It is the responsibility of the Offeror to indicate what submission(s) the reference is supporting. After completing Section A of the questionnaire, the document shall be electronically sent directly to the individual providing the reference.
Instructions to Individual Providing Reference (Section B):
In order to assess potential vendors under this Department of Commerce (DOC), National Oceanic and Atmospheric Administration (NOAA), Acquisition and Grants Office (AGO), Professional and Technical (ProTech) Services procurement, the government is requesting past performance information with respect to each Offeror. As a reference for work performed by the firm identified in Section A of this questionnaire, your candid response will assist in the evaluation process. Please complete Section B of the questionnaire as thoroughly as possible, and submit to ProTech.Weather@noaa.gov by 3:00 PM local time on September 5, 2018. The subject line in the submission email shall clearly indicate: ProTech Past Performance Questionnaire Submission for <Company Name> and the message shall originate from the reference’s government or commercial email system.
In addition to this questionnaire, you may receive a follow-up phone call to confirm or clarify information. This is a major NOAA acquisition and your input is greatly appreciated. If you have questions regarding this questionnaire, please submit inquiries to ProTech.Weather@noaa.gov.
