SCOPE OF WORK: HHCC QUARTERS CONSTRUCTION, PROJECT PH19KC01Q3
I. DESCRIPTION
A. The purpose of this project is for the completion of additional residential quarter units for the staff members for the Hopi Health Care Center (HHCC). The proposed project will construct four new staff quarters buildings, each with four one‐bedroom units, for a total of sixteen (16) new residences. The work consists of utility infrastructure, pavement, a lift station, the units, and the furnishing of each unit. The project is located at HHCC, Highway 264 Mile Post 264, Polacca, AZ, 86042+4000.
B. The Contractor shall be responsible for all equipment, parts, material, labor, travel, per diem, Tribal Employment Rights Office (TERO) fees and other expenses required to complete the project as described in this scope of work.
II. SUMMARY OF REQUIREMENTS AND OBJECTIVES
A. Refer to Attachment 1a for project drawings. Refer to Attachment 1b for project specifications. Additional requirements and objectives for the project are presented in this scope of work.
III. DELIVERABLES: CONSTRUCTION
A. Construct the Quarters in accordance with the attached plans and specifications.
B. Provide comprehensive As‐Builts/Project Record Drawings/AutoCAD Drawings
1. One hard copy set of full size sheets (24” x 36”) with updates and modifications clearly noted in legible handwritten drafting quality text.
2. Project Record Drawings: Maintain one complete full‐size set of contract drawings.
Clearly mark changes, deletions, and additions to show actual constructed conditions. Keep record drawings current. Certification of accuracy and completeness will be required for monthly payment request. On completion of the total project, submit complete record drawings including AutoCAD drawings.
IV. GENERAL CONDITIONS
A. Codes and Standards. The Contractor shall adhere to the following code requirements for work performed, including testing and inspection:
1. International Residential Code (latest edition)
2. Occupational Safety and Health Act – CFR1926
B. Project Safety
1. All work will comply with Occupational Safety and Health Act – CFR 1926.
2. Contractor shall supply all required worksite safety equipment.
3. Contractors will be required to submit and adhere to the following items including but not limited to:
a) SDS construction binder for all products used onsite.
b) Project specific site safety plan.
C. Submittals: Furnish submittals for all building materials, fixtures and appliances. Provide in scanned PDF format for formal approval.
1. SDS Sheets for all products.
D. Quality Assurance/Quality Control.
1. The COR is the Government representative who will inspect and accept the work and process invoices in IPP.
2. All materials shall be installed per manufacturer’s instructions and all applicable codes.
3. Housekeeping will be accomplished on a daily basis. All debris and trash generated by this project will be disposed of in a trash dumpster provided by the owner for this project at no extra cost to the contractor/installer.
4. Qualifications: All electrical, plumbing, HVAC, cabinet and trim work shall be performed by an Arizona licensed contractor specializing in the specific trade required.
E. Execution
1. Construction activities shall be coordinated with the COR and Hospital Facility Management Staff to ensure that there are no detrimental impacts on the day to day operations of the Hospital or staff safety.
F. Inspection
1. The Government reserves the right to inspect all aspects of work performed, including hiring a third party inspector to verify proper installation and operation.
2. Contractor is required to demonstrate full and complete operation of all work performed.
3. Final acceptance will be based on an acceptable final inspection, including final verification testing of all material and workmanship.
G. Project Schedule
1. The contactor shall provide a detailed schedule indicating the date that each phase of construction will be completed.
2. Facility day time hours of operation are 8:00 AM – 5:00 PM, Monday through Friday (excluding Federal holidays). Work outside of these hours must be submitted in writing to the Facility Manager for approval not less than 48 hours in advance of the proposed modified schedule.
3. Project schedule shall be in a Gantt chart format through Microsoft Project or approved equal.
H. Performance Period: The Government shall allow 248 days from the date of NTP.
I. TERO, Insurance and Bonding Requirements.
1. Contractors are required to contact the Hopi Tribe’s Tribal Employment Rights Ordinance (TERO) Office upon receipt of notification of award. Compliance with the tribal employment ordinance is required prior to starting work on any project performed on Tribal Land including payment of associated TERO fees/permits.
2. Obtain a business license from the TERO Office. The general contractor and each subcontractor are required to obtain the business license on each project. Submit required paperwork and pay business license fees.

J. Certified Payroll shall be submitted to the Contract Officer through the Project Manager.
Subcontractors are required to ensure that all non‐exempt employees for this work are compensated in accordance with the approved Davis Bacon Wage Act and proper certified payroll reports are completed and submitted.
K. Final Payment.
1. Final Payment will not be authorized until all work is complete (including punch list), the final inspection has been made and Warranty Documents for the Quarters and furnishings (appliances) have been received.
