About this file

This file is Exhibit 11 of the State of Ohio Department of Administrative Services Medical Plan Third-Party Administrator RFP, establishing the methodology and attestation requirements for reporting provider discounts. The exhibit specifies that offerors must provide historical provider discounts for each three-digit ZIP code in their service area, calculated using claims data incurred January 1, 2025 through December 31, 2025 and paid through February 28, 2026. Data must be separated into five service-type groupings: inpatient facility charges, outpatient facility charges, professional and other services, out-of-network contracted services, and non-contracted services. The analysis must include only commercial group claims and must incorporate all contracted and non-contracted provider claims, high-cost claims, claims from rental networks, and claim adjustments such as withholds and pay-for-performance arrangements.

Offerors must exclude Medicare Supplement, Medicare Advantage, Individual, Medicare, and Medicaid claims, as well as claims for members age 65 or older, prescription drug claims, dental and vision claims, claims subject to coordination of benefits where the organization is secondary, capitated arrangements, surcharges, network access fees, and claims involving custom network arrangements established for specific customers. All provider discount submissions must be attested by the offeror's chief actuary, who certifies the data's accuracy and compliance with the specified procedures. The attestation form requires the carrier name, actuary signature, printed name, title, and date, along with disclosure of any deviations from data specifications and any reliance on third parties in completing the data submission.

Data Specifications

Time Period

Include all medical claims incurred January 1, 2025 to December, 31, 2025 and paid through February 28, 2026 for the Historical Discounts.

C. Professional and other services (in-network contracted only, including professional charges associated with facility claims);

D. Out-of-network contracted services (This includes services where the provider is contracted with the health plan and benefits are paid at an out-of-network benefit level. An example of this is when a plan has a "wrap" network for out-of-network claims). or when a one-time negotiation is completed; and E. All non-contracted services (in-network and out-of-network), which would include claims paid as a percentage of Medicare or other specified levels such as Reasonable &Customary.
