About this file
This document outlines the qualification requirements for sources interested in manufacturing a Television Monitor for use in the AN/DSM-157 Guided Missile Test Set. Sources must notify the contracting officer of their intent to qualify and certify that they have the required manufacturing, testing and inspection capabilities. Sources must also verify possession of the necessary technical data package and develop any missing procedures for government approval. Sources are required to identify material sources and internal process standards, and define inspection and test procedures including functionality testing with the AN/DSM-157 system. Sources then prepare and conduct a qualification test plan before submitting results to the government for review. If qualified, the source will provide a sample unit and documentation for safety certification. The process is estimated to take 180 days. Once qualified, the source will be listed as approved for Solicitation FA8212-21-R-1164 from the Department of the Air Force seeking the Television Monitor for the AN/DSM-157 system.

MANUFACTURING SOURCE QUALIFICATION STATEMENT
(Public Law 98-525 10 USC SECTION 2319)
Stock Number (NSN): 4935-01-532-1694 Part Number (P/N): 6457314-2 Nomenclature: Monitor, Television Application: AGM-65 Maverick Air-to-Ground Missile
SECTION C. QUALIFICATION REQUIREMENTS THAT MUST BE SATISFIED
TO BECOME A QUALIFIED SOURCE AND QUALIFICATION WAIVER
REQUIREMENTS.
1. Pre-qualification Notice. The offeror shall notify the Small Business Office or, if responding to a solicitation, the contracting officer in OL:PZA of intent to qualify as a source for this item.
2. Verification of Manufacturing, Test and Inspection Capabilities. The offeror shall certify to AFLCMC/EBHGA, Maverick Missile System Program Office, the availability of the required manufacture, test, and inspection facilities and equipment, or provisions for their acquisition or utilization from another source.
When the offeror proposes to acquire these items, the offeror shall provide documented equipment orders or similar documentation for government inspection. The offeror shall make their facilities available for evaluation and inspection by the government.
3. Data Verification. The offeror shall verify possession of a complete data package by providing a list of applicable drawings, procedures, and specifications currently in their possession related to the manufacture of the Television Monitor.
This drawing package must be sufficient to show in thorough technical detail how the Television Monitor shall meet the interface requirements need to be used with the AN/DSM-157, and also how it will be manufactured and tested. The offeror shall develop those procedures for review and approval by AFLCMC/EBHGA.
In addition, the package must contain an acceptance test procedure designed to be a final functional checkout of manufactured devices sufficient in detail to ensure all operational functions meet specification. When the offeror proposes to use industry standards during manufacture of the Television Monitor, the offeror shall document the differences between the industry standard and the military standard, and state why those differences do not adversely affect the ability of the Television Monitor to pass the final functional check-out. AFLCMC/EBHGA shall determine if the industry standard is acceptable for use in the manufacture of the said Television Monitor.
4. Process Verification. The offeror shall identify sources for materials and standards for internally used processes. The offeror shall further define/provide written procedures for the utilization of any special test or inspection facilities and/or equipment. This documentation shall also include a diagnostic procedure showing the process the Television Monitor being manufactured would undergo.
The manufacture process shall show how the offeror shall test, inspect, diagnose, repair and perform a final functional checkout. The ability to test Hardness Critical Items (HCI) will be required in cases where HCI parts are involved.
Development of special test procedures not available from the government is required. The offeror shall develop those procedures for review and approval by
AFLCMC/EBHGA.
5. Test Evaluation. The offeror at their own expense, shall prepare and submit to AFLCMC/EBHGA for their prior approval, a qualification test plan/procedure detailing how they intend to verify compliance with all performance, environmental, mechanical and quality assurance requirements identified in the data package. After completion of the approved qualification testing, they shall be required to submit a complete test report of the results to AFLCMC/EBHGA for their review and approval. The test report shall verify compliance with all performance, environmental, mechanical, and quality assurance requirements identified by 86M-3132, Cage 07878, Critical Item Fabrication Specification for TV Monitor, and 6457314-2, Cage 0VWL8, Source Control Drawing, Monitor, Television, and other applicable drawings. Interfacing and functionality with the AN/DSM-157 GMTS, must also be demonstrated in the test report as well as verification that the Television Monitor has no impact on the Electromagnetic Radiation requirements (paragraph 3.3.2) specified in WS319-TS001A, Cage 07878, Prime Item Development Specification for Test Set, Guided Missile, AN/DSM-157. In addition the test report shall contain detailed documentation describing the testing procedures during manufacture. This data shall describe in detail the point(s) in the manufacturing process that testing occurred, and if failures were noted, the corrective action taken and the results of the retest.
The government retains the option to inspect the manufacturing and testing process, including on-site witnessing of any or all manufacturing and testing activities. To allow accomplishing this, the offeror shall notify the government at least 14 business days in advance of the occurrence of any testing that will be used as a basis for qualification. The offeror's facilities shall be made available for government inspection during these tests.
6. Qualification Article. Upon satisfactory compliance with the provisions of paragraphs 1 to 5 above, the offeror shall provide AFLCMC/EBHGA with a qualification article, which shall be production equivalent, as well as all necessary documents to support certification by the USAF Non-nuclear Munitions Safety Board (NNMSB, reference AFI 91-205). Upon successful completion of qualification testing and certification by the NNMSB, the Television Monitor will be considered consumed and not returned to the offeror. However, successful completion of the qualification testing and NNMSB certification does not guarantee any contract award. If the offeror is deemed qualified and awarded the contract, a post-contract first article exhibit may be required to verify production capability.
7. Sensitive Military Technology. Not applicable
8. Time Completion Estimate. It is the estimate of the design control activity that completion of this qualification effort should require 180 days. This is based on the complexity of the Television Monitor requirements for use in and with explosives, and other factors. This is not a deadline but a notification to a potential offeror of the time we believe will be required. Events that occur during the qualification process may cause this time requirement to increase or perhaps decrease, depending on individual circumstances.
9. Time Limitation. An offeror may not be denied the opportunity to submit and have considered an offer for a contract if the offeror can demonstrate to the satisfaction of the contracting officer that the offeror (or its product) meets these standards for qualification or can meet them before the date specified for award of the contract. The award will not be delayed to provide the vendor with an opportunity to demonstrate its ability to meet the standards specified in this source qualification statement.
10. Evaluation of Proposals Not Previously Qualified. The offeror must be fully qualified in order to be eligible for a contract award. Therefore, the offeror must fulfill all of the requirements stated, in writing, in the source qualification statement before the date specified for award of the contract.
The government reserves the right to award a contract at the time specified for award, to a qualified source even though other sources may not have finished qualifying.
11. Waiver. An offeror who has had previous experience in the manufacture and qualification of items, which can be correlated with this product, may apply to AFLCMC/EBHGA for a waiver of paragraph 2 tasks in the above stated qualification requirements.
a. The burden of proof for written inputs is on the offeror.
AFLCMC/EBHGA will not pursue authenticity verification of claims made by the offeror of product manufacturing experience with other Government or non-Government agencies. Unsubstantiated claims will not be considered in the waiver analysis process.
b. This waiver will be granted if and only if AFLCMC/EBHGA can establish the qualifications of the offeror through the evaluation of written inputs from the offeror, or from previous knowledge of the offeror's capabilities, or from previous experience with the offeror on similar item acquisitions.
If there is any doubt about the offeror's capability, the offeror will be required to submit necessary requirements of paragraph 2. There is no
Stock Number (NSN): 4935-01-532-1694 Part Number (P/N): 6457314-2 Nomenclature: Monitor, Television Application: AGM-65 Maverick Air-to-Ground Missile guarantee of qualification by similarity. AFLCMC/EBHGA reserves the right to require a pre-qualification of all offerors.
12. Approval. Once the qualification requirements are met, the offeror will be listed as an approved source for this item on an Advanced Engineering Supplemental Order, used only by the DoD, to supplement Source Control Drawing, 6457314-2.
Approval, however, does not guarantee subsequent contract award.
