1. Safety Data Sheet Please include a Safety Data Sheet (SDS) for your product. Please ensure that the SDS has information for all significant components of the material. My SDS is attached: Yes or No .

2. Certificate of Analysis (COA) Please include a certificate of analysis that will be representative of the material you will be providing. Please detail the testing methodology and any assumptions that were made for this Certificate of Analysis. My Certificate of Analysis is attached: Yes_________ or No__________.

3. Sampling Plan Please detail the sampling plan you have for this material. Including when, where, and by whom it will be sampled. Please include any ISO or ASTM specifications/processes that would be performed on this material.

4. Sampler Please include the name of the sampler who will be performing the sampling. Please include any ISO certifications that sampler possesses.

5. Testing Plan Please detail the testing plan you have for this material including when, where, and by whom it will be tested. Please detail the methodology that will be used citing any relevant specifications.

6. Testing Laboratory Please include the name of the testing laboratory that will be performing the testing. Please include any ISO certifications that the testing laboratory possesses. As a note the sampler (under item #4, above) and the testing laboratory may be different companies but do not need to be.

7. Material Specification Please detail the specification of the material you are going to provide. Additionally, you must provide the certification requested below on your own letterhead.

9. Delivery Schedule Please detail your delivery schedule, to include the base period and option periods.

10. Economic Price Adjustment The clause at FAR 52.216‐2 Economic Price Adjustment – Standard Supplies, applies to this acquisition.

Adjustments shall be based on the Argus Metals published prices for Tantalite basis 30% Ta2O5. You may include a formula you believe would be appropriate using this recognized trade publication. Indicate your understanding that this clause will be used to determine the price of material delivered to the Government under the contract. Yes_______________ or No______________.

11. Proof of Conflict Free Status.

In accordance with Section 6.1.5 of the Statement of Work, submit proof of Conflict Free Status for the material to be delivered under an awarded contract.

12. Proposed Lot Size and Expected Number of Lots.

In accordance with Section 6.1.6 of the Statement of Work, the Contractor shall propose a lot size and the expected number of lots in its offer. This information will become a part of the contract upon award.

13. Chemical Testing from Previous Lots.

Submit chemical testing from previous lots of Ta2O5 material showing all components over 5 wt%.
