Statement of Work DLA Strategic Materials –Hammond Depot, Hammond, Indiana 46320

Forklift Rentals for Hammond Depot

1. Background & Objectives: DLA Strategic Materials is requesting (2) two Forklifts rentals for the period of (12) twelve months. The contractor will provide (2) two forklifts for a period of 12 months that meet the minimum specifications stated below. The contractor will provide pick up/ delivery and maintenance that will be included in the cost. The contractor will deliver the forklifts full of fuel but will not be responsible for fuel.

2. Place of Performance: The Hammond Depot –3200 Sheffield Ave, Hammond, Indiana 46327- 1003. The Hammond Depot is in need of additional forklifts to be used by employees to move commodities. Depot Manager would be point of contact for this proposal (219-937-5383).

3. Period of Performance: The Period of Performance shall be 365 days from the date of contract award

4. Requirement: The contractor will provide (2) 6k Pound Forklifts for a period of 365 days.

Pricing must include delivery, scheduled maintenance, emergency response (between hours of 7am- 4pm Monday-Friday) and delivery and pick up of rental equipment. The contractor shall pick up the equipment at the completion of the rental period, or when notified by the Government Point of Contact (POC). The Contractor will contact the Government POC in order to schedule the delivery and pickup of the equipment. The Hammond IN Depot Manager will be the Government Point of Contact for the contract. The Depot Manager may be reached at telephone number (219) 937-5383. Contractor shall contact the Depot manager three days prior to scheduled maintenance. Maintenance shall be performed at depot location.

5. Bio-Based Materials: The contractor shall utilize 100 percent (100%) bio-based products in performance of this (SOW) Statement of work. All bio-based materials shall meet the Original Manufacture’s (OEM) specifications.

6. Clean-up/ Disposal of Waste Materials: Upon completion of service(s) the contractor shall be required to clean-up the workspace and remove and dispose of any parts and debris from the government property. The contractor shall dispose of all hazardous waste in a manner that complies with applicable Federal, Indiana State and local laws and regulations. The contractor shall ensure that all “hazardous” waste is transported by a “Qualified Transporter”.

7. Spill Responsibility: The contractor is solely responsible for any and all spills or leaks during the performance of this SOW. Contractor shall clean up such spills or leaks to the satisfaction of the government that complies with all applicable Federal, Indiana State and local laws and regulations. The cleanup shall be at no cost to the Government.
