Quality Assurance Surveillance Plan (QASP) Defense Logistics Agency (DLA) Human Resources (J1) Training

DLA Pathways to Career Excellence (PaCE) Program Human Resources Career Field

1. Purpose (Statement about the requested training)

The Defense Logistics Agency (DLA), Pathways to Career Excellence (PaCE) Program training for developmental employees in the Human Resources career fields. DLA requires contractor support for the delivery of training designed to prepare participant for the journeyman-level in the HR career field.

2. Roles and Responsibilities

The following personnel shall oversee and coordinate surveillance activities.

1. Contracting Officer's Representative (COR): Karla Jones, J1 Training, Career Management Division (TI)

2. Alternate COR/WAWF Payment: Rhonda Croskey, J1 Training, Operations Support Division (TA)

Other Key Government Personnel - Yvonne Watson, Functional Contracting Representative, J1Training, Operations Support Division (TA)

Performance Requirements and Method of Surveillance

The below requirements are the key focus areas for performance; however, the company is expected to comply with all elements of the PWS.

The company will provide the name of the confirmed instructor not less than 15 calendar days prior to the start of the class. If the instructor was not previously approved or vetted for same course, the company will provide a copy of the instructor’s resume for review/approval by government.

COR will verify that contractor provides the name of the assigned instructor within required timeframe.

3.6

The company will provide the training link for virtual classes to the COR at least 7 calendar days prior to the start of class.

COR will verify that training link is delivered within required timeframe.

4.3.4

The typical class hours will be 8:00 a.m. – 4:30 p.m. (Eastern Standard Time) but may be adjusted for alternate time zones.

The total instruction period per day is expected to be at least 7 hours. If instruction period is less than 7 hours per day for any reason, the company will notify DLA within 24 hours. Student will be provided with a 60-minute lunch period and other short breaks, as appropriate.

COR will evaluate and may investigate any issues raised by customer.

4.8.2

The company will use 508 compliant course materials for up to 30 students. Minimum course materials will include participant guide and briefing slides and may be supplemented with class exercises and relevant handouts. All materials are subject to 508 Accessibility Requirements.

COR will evaluate and may investigate any issues raised by customer.

4.8.3 and 8.0

(VPAT) and course materials as part of their proposal for content and 508 review.

Course evaluations pushed to students via the Learning Management System (LMS) at conclusion of the training. Overall course evaluations must remain at a satisfactory level of 90%. Failure to do so will result in a less than satisfactory rating in the performance reporting system.

Course evaluations reviewed for satisfactory level by COR.

4.11

The company will provide the COR with an electronic copy of the completed course roster within 5 business days of course completion. The completed roster will reflect daily attendance record of each student.

COR will verify that roster is delivered within required timeframe.

6.2

The company will submit invoice through Wide-area Workflow (WAWF) within 5 days of course completion.

Primary COR will verify that invoice is submitted in WAWF within required timeframe. The primary COR will prepare evidentiary matter. Alternate COR/WAWF payment will accept/reject invoice.

7.1

METHODS OF SURVEILLANCE:

1. The Government reserves the right to perform inspections and surveillance to evaluate the Contractor's compliance to the contract terms and performance of the requirements in the PWS. The Government will make every effort to ensure that the surveillance methods described below are conducted in an objective, fair, and consistent manner.

1.1. Periodic Surveillance. This action occurs when the COR or other Government official observes a deficiency. Examples MAY include administrative incidents or delays. Regardless of where in the line-of-duty the COR observes contractual procedures not being followed, he/she has an obligation to document and report the deficiency to the Contracting Officer.

1.2. Customer Complaint Surveillance. This action is instituted when the COR receives a complaint from a stakeholder (stakeholder organizations may include Human Resources (J1), HR Operations, and J1 Training). The COR will obtain the complaint in writing and then investigate to determine its validity. If the complaint is deemed valid, the COR will immediately notify the contracting Officer for action. The COR will notify J1 Leadership, Functional Contracting Representative, and the complainant of the Government's response to their complaint.

2. Regardless of the surveillance method, the COR will always contact the contractor's manager or designated representative when an issue is identified and inform the manager of the specifics of the problem. The COR, with assistance from Other Key Government Personnel, will be responsible for monitoring the contractor's performance in meeting a specific performance work statement standard.

3. Surveillance results may be used as the basis for actions (to include payment deductions) against the contractor. In such cases, the Inspection of Services clause in the Contract becomes the basis for the CO's actions.

RATINGS.

Contractor performance will be rated using the Contractor Performance Assessment Report (CPAR). Metrics and methods are designed to determine if performance exceeds, meets, or does not meet a given standard and acceptable quality level. The rating scale below will be used to determine a positive, neutral, or negative outcome.

Performance Rating

Criteria

Excellent/ Outstanding

Performance meets contractual requirements and exceeds many to the government's benefits. The contractual performance of the element or sub-element being assessed was accomplished with no problems and contractor actions were highly effective.

Very Good Performance meets contractual requirements and exceeds some to the government benefits. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were effective.

Good Performance meets contractual requirements. The contractual performance contains some minor problems for which corrective actions taken by the contractor were satisfactory:

Marginal Performance does not meet some contractual requirements.

The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions.

The contractor's proposed actions appear marginally effective or were not fully implemented.

Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
