1.0. GENERAL.

1.1. SCOPE. The Defense Supply Center, Columbus (DSCC) is a 500-acre Federal site located at 3990 East Broad Street, Columbus, Ohio 43218-3990. DSCC requires occupational health services for approximately 6,500 individuals, not all individuals located on DSCC are covered under this contract. The Contracting Officer Technical Representative (COTR) will provide a list of organizations covered under this performance work statement (PWS) within ten days after award. The Contractor will not be reimbursed for services provided outside the organizations covered. Of the 6,500 individuals supported by this contract, approximately 200 are in an occupational medical surveillance program which is described in paragraph 3.2.5. The vast majority of services are expected to be delivered on site. Occupational medical services, other than those provided at the Nurses’ Stations, will be performed at a medical facility within a ten (10) mile radius of DSCC.

1.2. STAFF QUALIFICATIONS. All medical staff servicing this contract (including but not limited to clinics, laboratories, technicians, medical facility staff, and practitioners) shall possess qualifications derived from an accredited learning institution and be appropriately licensed in the State of Ohio. All staff who will be physically located at DSCC must be able to meet all security requirements as listed in Attachment 1.
