SP4702-11-R-0001
AMENDMENT 0002
QUESTIONS AND ANSWERS
1. Is any proprietary information that may be included in the RFP kept confidential or is it made public to all entities submitting bids?
Pursuant to the Federal Acquisition Regulation (FAR) http://farsite.hill.af.mil 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection Information, and other applicable law and agency regulations, Contractor proprietary proposal information is required to be protected from unauthorized disclosure. An offeror should mark only that information that is considered proprietary.
2. Is there a specific requirement, outline or format required for the submission of the RFP— i.e., number of copies, signatures, etc?
a. The first page of the solicitation and amendments instruct one copy to be provided.
b. Page 2 of the solicitation requires an affirmative statement that the offeror can meet the requirement to provide influenza vaccines for the events identified in Amendment 0001.
c. Page 2 of the solicitation restates the past performance requirement.
d. Pages 55 and 56 of the solicitation instruct applying the Source Selection legend to each page of the proposal and to provide past performance.
e. The proposal should be signed by a person authorized to act on behalf of the proposing entity and should be prepared in a logical manner for evaluation.
3. Are unit prices per month, per hour or something different?
Please note the asterisk next to “Quantity” and page 6 of the solicitation with the explanation “The quantities listed are estimates only for evaluation purposes. The Contractor will be paid only for actual work performed. Units are “each” unless otherwise noted. The unit price should be one month for those Contract Line Item Numbers (CLINS) with a quantity of “12 months” and one each for all other CLINs.
The quantities are estimates developed based on historical data but the Government is only obligated for actuals.
4. Can Medical Assistants participate in the administration of flu vaccines alongside nurses during on-site flu clinics?
Yes, Medical Assistants may participate in the administration of flu vaccines during on-site clinics.
5. Can examples of CLIN 0053 Health Referrals, PWS paragraph 4.3 be provided? Health Referrals may be requested when the medical provider does not have that capability in-house. For example – individuals who are required to have an annual audiometric test, but wear hearing aids may need to be sent to an audiologist.
6. PWS Paragraph 5.2.1, page 36 of the solicitation states the Government will not incur the costs of health screenings and CLIN0060 asks for a price; is the price to the government to be listed as “0?”
Yes, CLINs x060 shall reflect a cost $0. The CLINs are included for reporting/tracking purposes.
7. Will the contractor be responsible for completion of the OSHA300, Log of Work-Related Injuries and Illnesses?
The Contractor is responsible for completing the OSHA300 log as it relates to their employees. The Contractor is NOT responsible for completing an OSHA300 log for the installation.
8. For services such as Physical Exams (CLINs x005), does your current provider charge for each exam component in addition to the hourly provider fee?
For this solicitation, CLINs x005, Physical Exams should reflect all costs associated with the exam, including provider services.
9. Injury & illness examinations (CLINS x006)
a. For injury & illness examinations performed at the contractor’s clinic, would the same charge apply to all subsequent visits?
It depends on the medical services required. If during the first visit, all services provided are within the scope of the contract, Workers’ Compensation would not be utilized. If during the first visit, Worker’s Compensations was not required, then additional follow-up care may be billed under the contract. Once services are provided under Worker’s Compensation, all subsequent services are provided under Workers’ Compensation even if these services are available under the contract.
b. Are ancillary services such as injury X-ray’s, Durable Medical Equipment (DME) and surgical supplies billed in addition to the visit charge or does the visit charge need to be all inclusive?
X-rays would be billed under the contract using either CLINs x010 Extremity(ies) X-ray, or CLINs x012 Head/Spine X-ray. Durable Medical Equipment (DME) is not billable under the contract and would initiate the Worker’s Compensation process. General medical supplies such as ace bandages and other disposable items should be included in the visit charge unless they are included as part of the overhead costs.
10. Should the proposal be unaltered, including paragraph numbers? If the PWS states “at least 6 health promotion programs will be provided” and the offeror intends to provide 12, where is that information to be noted – should the PWS be altered or should the information be added in the Technical Proposal?
As stated in provision 52.212-2, Evaluation – Commercial Items, page 56 of the solicitation, to be determined technically acceptable, the offer must comply with all requirements. Using Health Promotions as the example: The PWS states "at least 6 (six) times per year." In offering 12 events, the proposal meets the requirements.
However, if offeror would like to suggest 3 (three) promotions, the offeror may submit an alternate offer in addition to a compliant offer. A cover letter or the Technical Proposal should explain two offers were submitted and they should be clearly identified. The alternate offer should identify where the proposal differs, reason/ for proposed change and cost differences.
11. How are CLINs x002, Additional Occupational Health Physician hours, to be addressed?
Does the government want the rate per hour inserted with 0 hours listed for a total amount of $0?
CLINs x002 refers to PWS paragraph 3.1 DOCTOR’S HOURs. The note at the end of the paragraph explains this CLIN is applicable when needed to support a heavy medical exam schedule. This Amendment revises the CLINs to allow offerors to reflect a per hour rate with zero hours in the quantity and total amount columns.
12. CLINs x004, Additional Occupational Health Nurse hours
a. How are CLINs x004 to be addressed?
CLINs x003, Occupational Health Nurse Services, are established to capture the yearly cost (total amount) to staff 2 nurse stations. It is determined that the minimum number of hours is 40 per nurse station with the total hours not to exceed 88 hours/week. This Amendment revises the CLINs to allow offerors to reflect a per hour rate with zero hours in the quantity and total amount columns for billing of hours exceeding 88 hours per week.
b. Does the government want projections based on the amount of time needed to administer flu shots?
No, all costs associated with performance of paragraph 5.1, SEASONAL INFLUENZA VACCINATIONS, shall be reflected in CLINs x051, Influenza Vaccine. This Amendment revises the referenced paragraphs in Section B for CLINs x051 from 3.2.3, 3.2.4 and 3.2.5 to 5.1.
c. Are these CLINs intended to include additional nursing staff hours needed during days when exams take place?
As stated in the answer to Question 12.a., these CLINs may be used to bill additional nursing staff hours when work time has exceeded 88 hours/week for both nurse stations.
d. Does it cover extra nursing hours of January, February and March when exams are more frequent?
REMINDER: Amendment 0001 deleted reference to the specific months for frequent exams. However, if nurse hours exceeded 88 hours per week due to numerous exams, CLINs x004 would be the appropriate place to invoice against for payment.
e. Does it cover nursing time for CPR training and health promotion events?
No. CPR training costs should be included in CLINs x055. No additional costs will be billed to the government for health promotions.
13. Can Certified Medical Assistants be used for Influenza mass vaccinations? How or where would time for utilization of medical assistants be billed?
Yes, Certified Medical Assistants may be used of Influenza mass vaccinations. All costs, including staffing, associated with influenza mass vaccinations should be reflected in CLINs x051.
14. May the contractor utilize Certified Medical Assistants in the Nurse Stations to obtain bloodwork, EKG's, vision testing for surveillance and other exams? This would allow more staff at a lower expense to the government, and would lessen wait time for employees having exams. If allowable, how or where would time for utilization of medical assistants be billed?
Medical Assistants may NOT be used during medical exams. As stated in response to Question 13., Certified Medical Assistants may only be used for Influenza mass vaccinations.
15. EXMINATIONS AFTER INJURIES AND ILLNESSES
a. For an injury determined by the ESOH office to be work related, is the first visit billed under the Contract and all subsequent visits billed under Workers' Compensation?
Reference PWS paragraph 1.9, EXAMINATIONS AFTER INJURIES AND ILLNESSES, it depends on the medical services required. If during the first visit, all services provided are within the scope of the contract, Workers’ Compensation would not be utilized. If during the first visit, Worker’s Compensations was not required, then additional follow-up care may be billed under the contract. Once services are provided under Worker’s Compensation all subsequent services are provided under Workers’ Compensation even if these services are available under the contract.
b. If the same case required services beyond the scope of the contract on the first visit (e.g., suturing), is the very first visit to be billed to Workers' Compensation?
During the first visit only the services beyond the scope of the contract would be billed to Worker’s Compensation.
16. Are there critical items beyond what is written for the Flu Vaccines that if the contractor cannot meet, a proposal should not be submitted? If yes, are they listed as such?
All services are required and all offerors submitting a signed proposal are stating they accept and agree to all the requirements. The Flu Vaccines was a highlighted since the events are to occur in the near future.
17. How involved is the Government during the transition period?
The Government plans to meet with the Contractor on a weekly basis for the first 60 days to assist with a smooth transition.
18. Attachment 2 cited in PWS paragraph 1.3, MATERIALS AND SERVICES, lists Government Furnished Equipment/Property for nurse stations. Is paper provided for copiers and fax machines?
The stations will have utility, telephone and internet. Fax machines, photocopiers are supplied with paper. Computers with Microsoft Office (including encrypted email) will also be provided
