Statement of Work Summary
This Statement of Work (SOW) outlines requirements for the Defense Logistics Agency (DLA) Disposition Services to procure new 4K electric forklifts for Continental United States (CONUS) locations. The contract is structured as a Firm-Fixed Price (FFP), Indefinite Delivery Indefinite Quantity (IDIQ) vehicle with a one-year base period and four optional one-year periods, not to exceed five years or 150% of the maximum estimated contract value. The contractor has six months from task order receipt to deliver each forklift, with possible extension upon written request submitted at least ten days before the performance period end date. All equipment must be new, manufacturer-approved items—not remanufactured or gray market—and must include all necessary cables, batteries, chargers, and accessories for operational readiness upon delivery. Each forklift requires a DoD item unique identification (IUID) in compliance with DFARS 252.211-7003.
The SOW specifies four line items (CLINs): CLIN 0001 is a 4K electric reach truck forklift with 24-inch load center, 3/4-degree tilt, 24-inch reach stroke, 42-inch forks, 13-foot minimum lift height, and solid rubber tires with maintenance-free batteries rated for 8-hour continuous use. CLIN 0002 is a stand-up 4K electric forklift with 48-inch tines. CLIN 0003 is a sit-down 4K electric forklift with 36-inch tines, open cab, 3-stage mast, and hydraulic fork positioner and side shifter. CLIN 0004 is a sit-down 4K electric forklift with 42-inch tines and 36-inch minimum carriage back rest. All models require visible LED strobe lights, audible reverse alarms, fire extinguishers, LED work lights, side mirrors, overhead guards meeting ANSI/ITSDF B56.1 standards, and full suspension seats. The contractor must provide one digital and one hard copy operator and maintenance manual per machine and conduct familiarization training within 14 business days of delivery (video training is acceptable). Training must cover startup/shutdown procedures, fluid identification and filling, preventative maintenance, safety, controls, operations, machine limitations, and tool/attachment usage. Equipment delivery occurs Monday–Friday during normal government working hours, excluding federal holidays. The contractor must notify DLA DS at least 14 business days before delivery using dladsopsmhe@dla.mil and Adam.English@DLA.mil. The warranty begins upon completion of familiarization training, and equipment must be operational within ten days of delivery. The contractor is solely responsible for spills or leaks resulting from its actions and must report incidents immediately and submit written follow-up reports within 24 hours. Invoicing is processed monthly through the DoD Wide Area Workflow (WAWF) system in accordance with DFARS 252.232-7003, with completed DD250 Material Inspection and Receiving Reports and data plate photographs required prior to government acceptance and payment.
