Statement of Work
PEG BOARD & METAL CARTS
Phoenix VA Health Care System 650 E. Indian School Road Phoenix, AZ 85012
1. General Objectives and Requirements A. Purpose: The Phoenix VA Health Care System (Agency) requires new metal carts and peg boards for the prep area upgrade to Sterile Processing Service for room 1425 to maximize space for instrumentation.
B. Authority for the Contract:
i. 38 U.S. Code § 7301.b “The primary function of the [Veterans Health] Administration is to provide a complete medical and hospital service for medical care and treatment of veterans, as provided in this title Functions of Veterans Health Administration and in regulations prescribed by the Under Secretary for Health]pursuant to this title.”
2. Background. With space limited throughout the medical facility we must maximize the space we have. Sterile Processing is working to re-organize room 1425 with new metal carts and peg boards for maximum space for the prep/ clean side in Sterile Processing.
3. Scope.
A. Maximizing Space
i. Adding new metal carts for storage of instrumentation and peg boards
ii. Color coated different types of instrumentation.
iii. Labeling shelving B. Installation services shall include:
i. Complete assembly of all metal carts and peg boards.
C. The Contractor shall cover all costs for parts, travel, and labor.
D. The Contractor shall notify the Agency his/her intent to come on-site before arrival.
E. The Contractor shall check-in with the Sterile Processing department upon arrival on station. The Sterile Processing department is located in Building, Room 1426.
F. The Contractor shall verify that the equipment that was serviced is fully functional before checking out.
G. The Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and confirms quality assurance tests results to Agency.
H. The Contract shall provide for the addition and/or removal of equipment and/or services.
4. Security Requirements.
A. Security Requirements for Unclassified Information: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract.
B. The vendor may not have physical administrative access to the equipment included in this contract that capture and process ePHI. The Contractor may not have access to ePHI while performing scheduled and unscheduled maintenance.
C. Privacy and HIPAA Focused Training Web-Based (WBT): The official privacy training for VA contractors is VA Course No. VA10203. This training is not needed when contractor needs access to medical facility for two or more consecutive days, or who will require access to VA information or systems in the (e.g. CPRS or VistA Web). Course is available from the TMS website at: https://www.tms.va.gov. Certificate of training must be provided to COR upon request.
6. Risk Control. The Contractor shall be escorted by the Agency. The Contractor may need passes from the on-station Police Department.
7. Performance Monitoring. The Agency shall oversee work done by the vendor and accept the quality of work as appropriate.
8. Other Pertinent Information or Special Considerations.
A. Possible Follow-on Work: The Contract shall provide for the addition and removal of equipment.
B. Packaging, Packing and Shipping Instructions: All parcels and packages shall come through the Agency’s warehouse to be distributed. All parcels and packages shall ship overnight.
C. Inspection and Acceptance Criteria: The Agency shall sign off on the work completed if service is determined to be satisfactory.

· Cabinets are of unitary design.
· Cabinet Housings are made of all steel construction utilizing commercial grade steel.
· Cabinets have four holes/knockouts per side and 2 per back, for side to side and back to back bolting applications.
· Each drawer has rating of 440lbs capacity.
· Drawers are made of all steel construction utilizing commercial grade steel.
· All drawers have a “fail-safe” drawer stop system to contain working drawers without damage.
· All drawers extend 100% out of the housing exposing all contents to reach and visibility.
· Each drawer front has a drawer pull and label system and accepts a label 1-1/8” high.
· Stainless Steel worksurface is 18 ga 340 SS.
