STATEMENT OF WORK (SOW)
Maintain Roads and Parking, Surface Treatment and Markings FY25
Mountain Home AFB, ID Capital Project No. QYZH 25-0016
Construction Only 14 April 2025
OVERALL OBJECTIVE
1. Mountain Home Air Force Base (MHAFB) requires annual pavements maintenance to roads and parking lots for traffic control and safety.
2. The overall objective of this program is to sustain and extend the service life of pavements by providing crack sealing, and surface treatments to MHAFB roads indicated in this Statement of Work and shown on the Drawings/Maps (attachment 3). New and/or refreshed pavement markings will be required for traffic control and safety particularly at intersections and pedestrian crosswalks.
3. The project will be performed in accordance with this Statement of Work, Specifications (attachment 1), and applicable International Slurry Seal Association (ISSA), Federal Highway Administration (FHWA), and Idaho Transportation Department (ITD) requirements and specifications.
PROJECT DESCRIPTION
1. Location: The work site is located on Mountain Home Air Force Base (MHAFB), Idaho, 83648, at multiple street locations as shown on the attached Drawings/Maps.
2. Completion Period is 90 calendar days. The intent is for all work to be completed in one continuous effort, so the project is 100% complete within the time stated.
3. Government Points of Contact (POC):

GENERAL REQUIREMENTS
1. The Contractor shall provide all construction services, labor, materials, and equipment necessary to complete the requirements identified in this scope and the project design drawings and specifications.
2. Provide adequate professional supervision, quality control, environmental, and safety measures to ensure the accuracy, quality, timeliness, delivery, and completeness of the work during the execution of the construction.
3. The project is comprised of surface treatment of roads with Slurry Seal Type II, crack cleaning and sealing, and new markings on treated roads to include lane separation lines, crosswalks, directional mailto:jeremy.hemington.1@us.af.mil mailto:james.byron.3.ctr@us.af.mil mailto:oladimeji.oladeji@us.af.mil arrows, and parking lot markings. All markings will be replaced as they were found. Estimated quantities provided by the Government in project documents are considered informational only. The Contractor must validate quantities and all features of work necessary to deliver a complete and functional project. Estimated quantities for surface treatment, crack sealing, and road markings are as follows:
a. ISSA A105 Slurry Seal Type III Surface Treatment: 157,250 SY
b. Seal joints and cracks ¼” to 1 ½” wide: 42,000 LF
c. Stop Bars, Crosswalks, Arrows, and Handicap spots: 10,400 SF
d. Roadway and Parking Lot, line markings, white or yellow: 123,400 LF
4. All cracks larger than 0.25 inches wide shall be repaired except for Falcon St. Follow requirements for crack sealing specified in UFGS 32 01 17.61 Sealing Cracks in Asphalt Paving. Do not seal cracks on Falcon as this road segment is rated as “Failed”; our intent is to use Slurry Seal treatment to help bind this road segment while we await future funding for full depth repair.
5. All asphalt roads as indicated within attached Map shall receive a Slurry Seal type II treatment. Slurry Seal shall be installed per the International Slurry Seal Association specification ISSA A105 and UFC 3 250 03, Chapter 4. Prepare roadway surfaces in accordance with A105 including protection of manholes, railroad tracks, emergency barriers, spike strip, valve boxes, drop inlets and other service entrances and items from slurry seal.
6. All markings (except for parking lots) shall retroreflective as specified in UFGS 32 17 23 Pavement Markings to match existing markings. Obliteration of existing markings is not required. The intent is for markings to be placed after all slurry seal and crack seal operations have been completed on each road segment indicated within the attached map.
7. Traffic Control: A traffic control plan, project phasing plan, and schedule shall be submitted for approval prior to any work being performed. All facilities along roads and streets receiving work shall have continuous and uninterrupted access to the maximum extent possible. All traffic control shall conform to the Federal Highway Administration (FHWA) Manual on Uniform Traffic Control Devices
(MUCTD).
a. Flaggers shall be utilized during milling operations to ensure traffic flow around equipment.
Flaggers should move with the machines during operations. Include this in the Traffic Control Plan submittal.
b. Traffic Control Plan (TCP) shall be submitted for Government review and coordination with agencies including the: Fire Department, Security Forces, Ambulatory Services, Emergency Management, and Public Affairs. Work may not begin until the Government approves the contractors the TCP. The Government requires 30 days for review and approval of the contractors TCP.
c. Once the TCP is approved, the contractor may place traffic control signs at the roads, intersections, and detour routes one week prior to starting work at the areas affected.
d. Work within the Main Base Gate and the Grandview Gate areas must not be done during “rush hour” periods 0630 – 0900 hrs and 1500 – 1700 hrs, Monday – Friday, holidays not included.
One lane for entry and exit must remain open during work operations. Grandview Gate has only one exit lane, it would be preferable to perform this work on the weekend. Otherwise, contractor will have to route outbound traffic through the gate entry implementing all appropriate traffic control measures.
e. Main Base Gate in-bound lanes through the guard house area cannot accommodate large trailers.
COORDINATION AND AUTHORITY
1. Under no circumstances shall anyone other than the CO have authority to make any commitments or changes on behalf of the Government with the contractor that affect price, quality, quantity, delivery, or any other terms and conditions of the contract, nor shall any other Government representative in any way direct the contractor or its subcontractors to operate in conflict with the contract terms and conditions.
2. All work shall be conducted under the general direction of the CO and shall be subject to daily inspection by the Government at any time. References to “Contracting Officer” in the scope of work, specifications, drawings, etc. may be interchangeable with Contracting Officer’s Representative (COR), Contracting Officer’s Technical Representative (COTR), Project Manager (PM), Inspector, Subject Matter Expert (SME), “shop”-specific personnel, or other Government representative(s) that are assisting the CO with technical monitoring or administration of contract requirements, identified, appointed, or delegated certain responsibilities by the CO.
3. The Contractor shall confirm with the CO and PM who is/are the responsible Government representative(s) for the Contractor to successfully coordinate all construction activities. When a work task specifically requires the presence/witness of a responsible Government representative, the Contractor shall ensure the presence/witness is coordinated not later than one week before the work task is scheduled to be completed.
4. The Contractor shall hold weekly coordination meetings to discuss current status, future scheduled work
(3-week lookahead), submittals, Requests for Information (RFIs), safety, coordination requirements, delays, etc. Meeting agendas and meeting minutes shall be provided by the contractor to all attendees.
WORK PROGRESS AND SCHEDULE
1. Upon award, the Contractor shall proceed with securing payment and performance surety bonds. No other efforts shall commence until the CO issues a Notice to Proceed (NTP) except for attendance of a preconstruction conference. The conference will be held with the CO and PM prior to the start of construction at the work site.
2. Upon receipt of the NTP, the Contractor shall proceed with all other efforts required by the contract. All preconstruction submittals shall be approved prior to commencement of any construction at the work site, unless otherwise authorized by the CO.
3. Work Schedule: The Contractor shall submit for approval a baseline work schedule to the Government before start of work. Refer to the specifications for schedule requirements in addition to those contained within this SOW. The Contractor shall provide timelines and critical path items showing an understanding of the project requirements and capability in achieving the desired period of performance provided in this SOW.
4. Working Hours: Normal working hours for the Government PM and CI are between 6:30 am to 4:30 pm local Monday thru Friday; however, any reasonable time between 06:00 am to 6:00 pm local is allowed for the Contractor to implement work for this project.
5. Work outside normal working days/hours must be coordinated with the PM and/or CI at least one week prior to the work occurring and be approved by the Contracting Officer.
a. Work on weekends and/or holidays shall not occur without two weeks advance notice to the PM and/or CI and approval by the CO.
6. The scheduling and reporting of percentage of completion contemplated by FAR Clause 52.232-5
Payments under Fixed-Price Construction Contracts and 52.236-15, Schedules for Construction Contracts, shall be accomplished on or equivalent to an Air Force Form (AF) 3064, Contract Progress Schedule, and MHAFB AF Form 3065, Contract Progress Report.
7. The project schedule shall incorporate, as applicable, milestone events specified in the contract, including, notice to proceed, substantial completion, milestones related to specified work phases and site restrictions, Contractor-defined milestones to identify target dates for critical events based upon the Contractor's chosen sequence of work, and depict all major activities necessary to complete the work.
8. The Contractor shall also prepare and submit for approval a cost breakdown of the Contract price, to be referred to as the “schedule of values”, assigning values to each major activity necessary to complete the work. Values must include all direct and indirect costs, although a separate value for bond costs may be established. The schedule of values must contain sufficient detail to enable the CO to evaluate applications for payment.
9. If the percentage completed for the monthly reported period is less than the approved scheduled percentage the contractor shall provide the CO with an explanation for falling behind. If progress will not be regained within the next monthly reported period, the contractor shall provide the CO a supplementary schedule for approval demonstrating how progress will be regained.
10. Progress payments may be made up to substantial completion (95%) and after a pre-final inspection has occurred with the CO. The final progress payment of 5% (Final Invoice) will only be made following completion of any remaining punch-list items and final inspection, submission of the final progress report (100%), any remaining submittals, records, drawings, payrolls, Standard Form 1413s, release of claims, and other closeout documents, and turnover of Government property (materials, equipment, keys, etc.).
11. Weekends, Federal holidays, and any associated ‘AF Family Day’ are non-workdays and decrease the availability of Government personnel for coordination. Federal holidays are: New Year’s Day (and associated Family Day); Birthday of Martin Luther King Jr; Inauguration Day; Washington’s Birthday/President’s Day (associated Family Day); Memorial Day (associated Family Day); Juneteenth National Independence Day; Independence Day (associated Family Day); Labor Day (associated Family Day); Columbus Day; Veterans Day; Thanksgiving Day (associated Family Day); and Christmas Day (associated Family Day).
12. Further decreased availability of Government personnel shall be anticipated during the workweeks of the
Thanksgiving, Christmas and New Year’s holidays. It is the responsibility of the Contractor to adequately plan for and schedule all coordination necessary (i.e. base/site access, submittals, construction) in advance of known non-workdays.
13. Work on weekends and/or holidays shall not occur without two weeks advance notice to the PM and/or
CI and approval by the CO.
8. Water will be available from an on-base Government hydrant located at the of the base water tower and may be used at no cost to the Contractor.
a. The Contractor must furnish equipment and labor for delivery of water from the water source to the job site.
9. Wastewater and Solid Waste Disposal for asphalt, paint, and debris removal operations must comply with the following:
a. Follow requirements in specification 01 57 19 Temporary Environmental Controls provided.
b. All asphalt, aggregate, subbase, dirt, and paint removal debris collected from crack cleaning/routing, asphalt patching, and paint removal operations shall be disposed of in a permitted sanitary landfill that will accept the waste in accordance with all applicable environmental regulatory requirements.
10. The Contractor shall hold weekly progress meetings to discuss status, the next weeks work schedule, and any coordination requirements.
11. Prospective contractors are to base their bids on the Government quantities provided herein. As work is performed, the contractor is to track each of the quantities and report weekly. If it appears those quantities are going to exceed the Governments estimates, the Contractor and Government will discuss how to proceed.
12. The Government requests a unit cost breakdown and total lump sum cost for each of the objectives listed below.
SUBMITTALS
1. The Submittal Register is provided within the specifications. The Contractor shall submit materials to the CO for review and approval with the following information (at a minimum): manufacturer's product brochures and catalog data for all equipment, components and materials to be installed. All material submittals shall be original vendor information, not copies.
2. All submittals will be coordinated through the CO using an AF Form 3000. The CO may request submittals in addition to those specified when deemed necessary to adequately describe work covered.
3. Each submittal is to be complete and in sufficient detail to allow ready determination of compliance with contract requirements. If the submittal is incomplete or otherwise does not provide the Government with sufficient detail, the CO may request additional information to determine compliance; such requests shall not be construed as Government delay or entitle the contractor to any equitable adjustment.
4. The Government shall be allotted 14 calendar days after the date the Government receives any submittal
(initial or resubmission) to complete reviews. Upon transmission of the submittal to the Government the contractor shall confirm its receipt. The Contractor shall be allotted 14 calendar days for resubmission.
This allotment shall not be construed as any extension of the schedule or contract completion date.
5. Approval of a submittal shall not be construed as a complete check and indicates only that the Government does not otherwise object to the submittal. Approval shall not relieve the contractor of the responsibility to meet contract requirements or for any error.
SECURITY CONSIDERATIONS AND BASE ACCESS
1. All workers shall be subject to the base rules of entry and exit and subject to search at any time.
2. The Contractor shall cooperate fully in all security matters, which may arise relating to this contract and/or base activities.
3. It is prohibited to carry/use weapons on the site without prior USG approval. If approval is granted, personnel will abide by MHAFB policies for authorized use.
4. Cell phones are authorized on the site.
5. The base Visitor Control Center (VCC) must receive a Security Forces Squadron (SFS) Form 30 for each contractor and subcontractor employee that needs access to the base not later than two weeks prior to construction start or otherwise before a valid need for base access. The Contractor shall submit all project SFS Form 30s to the CO (Sponsor) for processing to the VCC prior to the two-week lead time.
No consideration shall be given to the Contractor for delays or cost increases experienced due to failing to request base access in a timely manner, nor due to the denial of base access to a contractor or subcontractor employee following the Government-performed background check. 6. The 366th Fighter Wing Commander may at any time deny or restrict entry to or exit from MHAFB.
Typically, these occurrences are in the interest of heightened security conditions and public safety concerns. Contractors and their subcontractors shall anticipate the potential for minor entry or exit delays due to the nature of performing work on a military installation. Contractors shall further anticipate the potential for base closure or delayed reporting of Government personnel due to the expectation of or active poor weather conditions. Public notifications of base closures and delayed reporting of Government personnel due to poor weather conditions are available on the Mountain Home Air Force Base Gunfighters Facebook page.
7. Inappropriate actions and/or violations may result in the immediate removal of the offender from the base.
PERSONNEL
1. The Contractor shall employ sufficient management and contract administration resources, including personnel responsible for project management, field superintendence, change order administration, estimating, coordination, inspection, and quality control, to ensure proper execution and timely completion of work.
2. The Contractor shall designate a principal of the firm or other senior management official to provide executive oversight and problem resolution resources to projects for the life of the contract.

3. The Contractor shall employ and require its subcontractors to employ only qualified personnel to perform work required for each project.
4. The Government reserves the right to exclude, or remove from the worksite, any personnel for reasons of incompetence, carelessness, or insubordination, who violate rules and regulations concerning conduct on federal property, or whose continued employment on the worksite is otherwise deemed by the Government to be contrary to the public interest.
5. For this project the contractor may combine, “triple-hat”, the positions of Superintendent, Site Safety and Health Officer, and Quality Control Manager, provided the individual meets the qualifications of each. A separate Environmental Manager is required.
6. The Contractor shall ensure their designees meets or exceeds the required qualifications of each role for which they are designated. Prior to construction, the Contractor shall provide the CO with written designation for each role, proof of qualifications for each designee, and work contact information (desk/mobile phone, email).
a. Superintendent: The project superintendent must be an individual with a minimum of 10 year of experience in construction with at least 5 of those years as a superintendent on projects similar in size and complexity. The individual must be familiar with the requirements of EM 385-1-1and have experience in the areas of hazard identification and safety compliance. The individual must be capable of interpreting the schedule and construction drawings.
b. Quality Control Manager (QCM): The QC Manager must be an individual with a minimum of 5 years combined experience in the following positions: Project Superintendent, QC Manager, Project Manager, Project Engineer or Construction Manager on similar size and type construction contracts which included the major trades that are part of this contract. The individual must have at least 2 years of experience as a QC Manager. The individual must be familiar with the requirements of EM 385-1-1and have experience in the areas of hazard identification, safety compliance, and sustainability.
c. Site Safety and Health Officer (SSHO): All SSHOs will have met the training and experience requirements in EM 385-1-1 and this contract.
d. Environmental Manager: The Environmental Manager must be an individual trained to adequately accomplish the following duties: ensure waste segregation and storage compatibility requirements are met; inspect and manage Satellite Accumulation areas; ensure only authorized personnel add wastes to containers; ensure Contractor personnel are trained in 40 CFR requirements in accordance with their position requirements; coordinate removal of waste containers; and maintain the Environmental Records binder and required documentation, including environmental permits compliance and close-out.
SAFETY
1. The Contractor shall meet OSHA standards and any other applicable safety laws and regulations. The Contractor must notify Safety, Contracting, and Civil Engineering on any safety emergencies.
2. The Contractor shall comply with the requirements of the Corps of Engineers Safety Manual EM-385-1- 1 at all times in the performance of this Contract. Hazards to the safe use of the premises due to the Contractor's work and/or equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed.
3. The Contractor shall submit with the Quality Control Plan, the name of the designated on- site safety persons responsible for ensuring that all safety and accident prevention provisions of this Statement of Work are implemented and enforced. The Contractor shall ensure his personnel receive safety training and briefings and that all safety inspections, practices, and procedures are within compliance. The name of the designated on-site safety persons, safety training, briefings, inspections, practices and procedures shall be included in the Quality Control Plan.
4. The Contractor shall indoctrinate each employee to ensure clear understanding of the following subject areas, but not limited to:
a. The purpose of the accident prevention program (i.e., to minimize hazards and prevent injuries).
b. A review of the hazards inherent in the work environment and the precautions to be taken.
c. Location of first aid and other emergency facilities.
d. A detailed description of what to do in case of injury, fire, or identification of a safety hazard.
e. Review of cleanup procedures.
f. Required safety equipment including earplugs, goggles, face guards, respirators, protective clothing, hard-hats, etc. (when and how to use).
g. Location of company safety rules (posting or handout).
h. Identification of the Contractor employees responsible for safety/accident prevention.
5. In addition to normal safety practices, the Contractor shall comply with the following requirements:
a. The safety of Contractor personnel on-site is the Contractor's responsibility.
b. All personnel shall receive an initial Air Force safety briefing, prior to site entry, and will comply with all requirements set forth in that briefing. All personnel shall comply with Air Force on-site safety procedures and the US Army Corps of Engineers Safety Manual EM-385-1-1 as directed by the CO.
c. The Contractor shall ensure that safety barriers/portable ladders, ladder extensions, or environmental covers are properly installed at access hatches and excavations.
d. Equipment shall be placed so as not to impede normal traffic in and out of the work area, nor to interfere with emergency exit from work area and off-site.
e. Extreme caution must be exercised to preclude personal contact with energized electrical circuits. Contractor shall comply with NFPA 70 and 72E for electrical safety.
f. Hard-hats shall be worn in accordance with EM-385-1-1 requirements and hard-toed safety shoes shall be worn by all personnel at all times on the work site. Other safety items such as ear protection, respirators and safety glasses, which are required by EM-385-1-1 and/or the manufacturer of products for specific hazards, shall be available and used as necessary.
g. All heavy equipment shall be equipped with backup alarm in accordance with EM- 385-1-
1regulations.
h. All vehicles shall be in safe operating condition. Vehicles and equipment not in compliance with EM-385-1-1requirements shall not be allowed on site.
6. First aid and medical requirements are as follows:
a. The Contractor shall comply with accident response requirements. These requirements are provided in a safety briefing by base personnel at the preconstruction conference. The Contractor shall keep these requirements on file at the working site for inspection by the Government.
b. The Contractor shall maintain an emergency first aid kit at the work site.
c. All accidents (employee injuries and vehicle, building, or equipment property damage, etc.), regardless of severity, shall be reported promptly to the CO.
CONTRACTOR LAYDOWN AND FACILITIES:
1. If the Contractor chooses to have storage or facilities on MHAFB, the Government will provide a laydown lot within a reasonable distance of utilities. The Contractor shall restore the land to its original condition within thirty (30) days of contract completion.
2. If the Contractor chooses to have a material storage yard MHAFB, the Contractor will be required to provide and install an approved fence to secure the area. The fence shall have a screen attached to conceal the materials inside.
3. If the Contractor chooses to have temporary facilities at MHAFB, the Contractor shall be responsible for any costs associated with their facilities to include mobilization, demobilization, trailers, skirting, utility lines, connections and site restoration. Contractor temporary facilities shall be located within the 50’ x 100’ lot provided on MHAFB. The facilities must conform to MHAFB’s color scheme, architecture standard and be in good repair. Contractor on-site offices shall comply with handicapped accessibility laws and regulations. See General Requirements for utilities availability.
4. The Contractor shall secure all supplies and equipment stored on the project site to discourage theft or damage. Contractor shall maintain the project site in a clean, safe and orderly manner. Protection and security for materials and equipment on site is the sole responsibility of the Contractor.
PROJECT CLOSEOUT
1. Pre-Final and Final Inspections:
a. The Contractor shall request a pre-final inspection with the CO at 95% completion. Any remaining work shall be minor in nature; major work elements shall be complete and the site otherwise ready for its intended use. The request shall be in writing (email is sufficient) at least three (3) working days prior to the date requested.
b. The pre-final will be led by the CO and attended by appropriate Government and Contractor personnel. The Superintendent shall be in attendance.
c. A punch-list of deficiencies will be generated that must be completed prior to a final inspection.
c. If any Contractor personnel are still working on major elements of the project, the CO will immediately cancel the inspection and direct the Contractor to reschedule when the work is complete.
d. The Contractor shall request a final inspection when all punch-list items have been completed/corrected. All punch-list items shall be completed/corrected and final cleanup of the site accomplished prior to the contract completion date.
