U.S. Coast Guard PAC-6 Building 51-4
Carpet Replacement Statement of Work (SOW)
1.0 Background
Due to the age and damage from wear and tear, need to be replaced with new carpeting (ref.
Variations IV Infinity Variations 4 | Modular | Carpet | Mannington Commercial). Replacing this carpet will keep flooring maintenance as easy as possible and maintain a clean and aesthetically pleasing atmosphere.
2.0 Scope of Work
Remove old and install 2,243 square feet of new carpeting in PAC-6 main office, conference room, two corridors, and reception area on the diagram below of Building 51, Bay 51-4.
Dismantle and relocate all conference room furniture to facilitate carpet installation and return to original position afterward. Remove and install cove base around carpet perimeter as required.
The contractor shall provide labor, materials and supervision to complete all work, and develop a timeline of completion, allowing for removal and replacement of cubicles and furniture in the main office to be coordinated accordingly.
PAC-63 is a United States government facility. Coast Guard personnel within the office will be responsible for removing all personal items from the space and floor. A separate company will remove all cubicles and furniture from the main office area. It will be the contractor's responsibility to dismantle, remove, and reinstall all furniture in the conference room, as needed, to perform the scope of work. Contractor shall take photographs of all existing furniture damage and issues and provide to the Coast Guard. Contractors shall be responsible for all damages that occur during removal/install and repair or replacement of all damaged items at the contractor's expense.
Carpet [squares] installed shall comply with UFGS 09 68 00, key characteristics for the carpet must comply with the following, but are not limited to:
Indoor Air Quality: CDPH Section 01350 End-Use Classification: 3.5 TARR traffic level ASTM D7330 or CRI Test Method 103 Flammability/Critical Radiant Flux: 16 CFR 1630 or ASTM D2859
Along with the removal/installation of carpet, the contractor shall remove/install new cove base in all existing locations- including non-carpeted areas for all Annex. The cove base to be used to replace existing conditions is as follows:

While presenting plans to replace carpet and cove base in sections, a plan needs to be presented ensuring that air quality within the annexes remains suitable for human occupancy and working conditions. The adhesives and concrete primer must comply with applicable regulations regarding toxic and hazardous materials, releasing no odors and VOC's. These chemicals must be water resistant, mildew resistant, nonflammable, nonstaining, and fast acting adhesives.
PAC-6 front hall contains an entrance to a secure space vital to PACAREA that will be in use occasionally throughout the project and shall be always accessible.
This summary is not all inclusive and delineates only those work items deemed as major to the contract. Ancillary work items and parts not covered, though necessary to attain the completed project, shall be included in the contract. Field verification by contractor is required. The working hours will be from 0700 to 1600, any work needing to be completed outside of prescribed work hours must be submitted to the CORs 24 hours in advance.
3.0 Site visit
All bidders are strongly encouraged to visit the site to field verify any/all quantities and measurements before submitting bids. 4.0 Designated Contracting Officer’s Representative
The designated Contracting Officer’s Technical Representative and Points of Contact for this project are: CWO2 Sheldon Williams, Sheldon.Williams@uscg.mil, (571) 607-7097 and ET1 Raina Memea, Raina.H.Memea@uscg.mil, (206) 820-0987. They will also be the points of contact for inquiries concerning any phase of the specification before or after award.
5.0 Conditions Affecting the Work
The Offeror should visit the site and take such other steps as may be reasonably necessary to ascertain the nature and location of the work, the general and local conditions which can affect the cost of the work thereof. Failure to do so will not relieve bidders from responsibility for estimating properly the difficulty or cost of successfully performing the work.
The government will assume no responsibility for any misunderstanding or misrepresentations concerning conditions made by any of its officers or agents prior to the execution of the contract, unless included in the request for quotes, the specification or related documents.

Communications given to the Supervisor shall be as binding as if given to the Contractor. Important communications will be presented and confirmed in writing.
5.0 After Award of The Contract
5.1 CONTINUITY OF FACILITIES OPERATION: Notify the Contracting Officer Technical Representative in advance of any shutdowns at least seventy-two hours prior to the disruption.
5.2 NOTIFICATION OF START AND COMPLETION OF WORK: The Contractor shall notify the Contracting Officer at a minimum of forty-eight hours in advance of the date they intend to commence work unless otherwise agreed upon. The schedule shall not exceed time limits current under the contract documents unless revised jointly by both the owner and the contractor due to conditions of the work and project. Once the work has been completed, the Contractor will notify the Contracting Officer to set a date for final inspection.
5.3 CHANGES IN THE WORK: No oral statement of any person other than the contracting officer shall in any manner or degree, modify or otherwise affect the terms of this contract. Changes in the work may be accomplished after the execution of the Contract, and without invalidating the Contract by a Change Order. The Federal Government defines the Change Order as a MODIFICATION to the CONTRACT. The Modification to the Contract shall be based on upon the agreement of the change in work by the Contractor, Contracting Officer’s Technical Representative, and the Contracting Officer. The Modification to the Contract is a written instrument describing the change in the work, appropriate cost adjustments, and any extension to the contract schedule time required due to the change in the work. This document is prepared by the Contracting Officer’s Technical Representative and submitted to the Contracting Officer for approval
5.4 At a minimum, Contractor shall:
5.4.1 Perform all work between the hours of 0700 and 1600 Monday through Friday. Contractor may coordinate weekend work with the KO.
5.4.2 Provide equipment, labor, materials, parts, facilities, and supervision to install all necessary hardware.
5.4.3 Accomplish work within prescribed calendar days after contract award.
5.4.4 Ensure that all work is accomplished with the best standard practices, and by workmen thoroughly experienced in the required trades. The work shall be supervised by experienced personnel and shall be satisfactory to the Contracting Officer.
5.4.5 Upon completion of work, Contact the Contracting Officer Technical Representative to arrange for inspection/acceptance of work. Clean work area of debris and equipment.
5.4.6 Upon acceptance of work, remove all waste (Molding, Carpet, Under-padding, Paste, Used Applicators, etc.) and dispose off-site in proper manner and restore the work area to a clean condition.
6.0 Government-Furnished Property
6.1 Permission to use the Building’s water, electricity, lighting, and HVAC for work is granted during working hours.
6.2 The Government will provide emergency telephone service only (911).
6.3 The Government will not provide waste or debris clean up or removal.
6.4 The Government will not be responsible for contractor equipment / tools security.
The Contractor must secure his own equipment.
7.0 Security Requirements
7.1 PAC-6 is a United States government facility with 2 secure spaces adjacent to the room in which work will be performed. Contractor does not require access to these spaces. All personnel accessing the PAC-6 office are subject to all applicable federal, state, and local laws, as well as Coast Guard regulations, particularly the Coast Guard Physical Security Manual. Enforcement of these regulations may include random personnel and vehicle searches to ensure compliance with United States Coast Guard security.
7.2 Accepted forms of identification for Contract Personnel:
The Real ID Act of 2005 established minimum security standards for license issuance and production and prohibits Federal agencies from accepting, for certain purposes, driver's licenses and identification cards from states not meeting the Act's minimum standards. To this purpose there are five states that are not in compliance with the Act nor have they received an extension to come into compliance with the act. Those states are Maine, Minnesota, Missouri, Montana and Washington State as well as American Samoa. Anyone under this contract, to include subcontractors, who holds a driver's license from any of states without approved licenses and require access to a military facility will be required to provide identification other than a driver's license to gain access. It is the responsibility of the Contractor to ensure that they are in accordance with the Real ID Act and any other requirements for base access. For the states of Washington and Minnesota an enhanced driver's license is acceptable to gain access.
7.2.1 Contract Construction Personnel: The Contractor shall provide a list of all on-site personnel, including sub-contractors (including second and third-tier subcontractors) and suppliers, to the Coast Guard. The contractor shall update this list when changes occur. Contractor personnel not listed may be denied access to the Coast Guard facility. Contractor personnel will be restricted to designated working areas. Any personnel violating this policy may lose access to the Coast Guard facility. Contractor personnel shall have photo identification at all times while working on Coast Guard facilities. Contractor and delivery personnel may be required to present personal photo identification to gain access to a Coast Guard installation. If identification does not indicate United States citizenship, Coast Guard Security may require proof of the legal right to work in the United States. Contractor and delivery personnel also may be subjected to an immigration status and outstanding criminal warrants check. A valid contract employee picture identification card. Company / Contractor identification must be worn and in plain sight at all times in the PAC-6 office.
7.2.2 Contractor work trucks must be marked or use magnetic signs advertising their company name and contact information.
7.2.3 All vehicles must be currently registered and have proof of insurance.
7.2.4 The Contractor shall provide the KO’s local liaison a list of persons to be on site at any given time and a list of the vehicles – make, model, year and color.
These must be provided in advance to ensure access to the base, and PAC-6 office. These personnel must also be escorted by a PAC-6 member.
8.0 Place of Performance
8.1 The Contractor shall install the carpeting on the first floor of Building 51-4 at Coast Guard Island Base Alameda. The address is:

8.2 Lay down area. The KO’s local liaison will provide the contractor with a lay down area for any equipment that will be left on site during the work. The contractor is responsible for any equipment left at the site. There is no secure space provided by the Coast Guard.
8.3 Vehicles are not permitted at any time to drive or park on the edge of the grass due to possibility of damaging pop-up lawn sprinklers. Any damaged sprinkler heads shall be reported and then replaced at Contractor’s expense.
9.0 Period of Performance
9.1 The Contractor shall complete the work no later than 14 calendar days after contract award.
9.2 The contractor is to schedule the work through the designated KO’s local liaison.
At the time of award the contractor shall coordinate a work schedule with the KO’s local liaison that least interrupts ongoing operations and other regular activities within this area.
