Repair LRS District Pavements

STATEMENT OF WORK

FOR

OPP ID #EEPZ230068

12 November 2024

1. SUBMITTALS REQUIRED

See Form 66 (attached)

2. SCOPE

The Contractor shall provide all plant, labor, equipment, transportation, and tools necessary to install a trenching duct in the area, mill 2” and overlay 2” of asphalt pavement within the LRS area, replace the existing asphalt loading dock area with concrete, re-seal the LRS parking lot, and repaint all existing pavement markings. Perform all work according to the Paving and Civil Works IDIQ specifications, Statement of Work (SOW), the Design Drawing Figures, and all related AF requirements and other codes and references.

3. DESIGN

3.1 The attached Design Drawing Figures indicates the area for asphalt mill and overlay and the area to replace with concrete.

3.2 Verify constructability and suitability of Design and SOW. Contractor shall field-verify all information related to existing conditions including: dimensions, elevations, grades, and code requirements for all elements of work.

3.3 Design Submittals shall include at a minimum the following Construction Documents (CD) or CD elements:

4. SCHEDULING

The Government does not expect work during other than normal business hours in order to meet project timelines. Normal business hours are 0730-1630 hours Monday through Friday.

Total Performance Period: 180 Days

4.1 AF FORM 3064/3065

The contractor’s schedule, AF Form 3064, will clearly list the performance period as indicated above.

Additionally, Progress Reports shall be completed & submitted for approval on a bi-weekly basis starting the first Monday after receipt of NTP. For any two week period that 0% work is accomplished, a Progress Report will still be submitted indicating such. If at any time during the performance of this contract, the contractor’s actual performance falls more than 5% behind the scheduled performance, the Contractor is required to provide written notice to the Government indicating what the issue is and proposing a way forward to correct the matter & ensure the contract is completed on-schedule.

4.2 SUPERINTENDENT

Contractor shall provide all required certifications & pertinent work history of the nominated Superintendent via Submittal. The Government’s approval of this submittal will document acceptance of the nominee as meeting all minimum requirements. In addition to being responsible for the day-to-day work on the jobsite, the Superintendent will also have the authority to make decisions on behalf of the Contractor. The Superintendent shall be on-site at all times when work is being performed. Should the Superintendent need to be absent from the jobsite (illness, injury, etc.), a written notice shall be provided by the Contractor to the Government indicating the length of absence, what, if any, critical path items will be worked during such absence, and whether the absence is expected to be temporary. If, due to extended absence, the Contractor or Government determines that a replacement Superintendent is necessary, a revised submittal will be provided for review/approval.

The replacement Superintendent must have qualifications and experience equal to or better than the existing Superintendent. If an approved Superintendent is unavailable during the performance of this contract for more than two weeks, a Suspension of Work may be issued until such matters may be resolved.

5. GOVERNMENT FURNISHED EQUIPMENT/MATERIALS

The Government will not furnish any equipment or materials.

6. AIRFIELD WORK

N/A for this project.

7. CONTROLLED AREA

Special security requirement apply to controlled areas. The area will require special coordination with base Security Forces. Before start of construction, the following information must be supplied to the Contracting Officer: start and completion dates, entry access list of all workers, address and contact numbers of all prime and subcontractors, and a detailed map of the construction area. All work must be scheduled in advance with the Government.

8. OTHER

8.1 No submittals will be accepted during the week of Thanksgiving and from the beginning of Christmas week until the end of New Year’s week.

8.2 Contractor must provide to the Contracting Officer the names of all employees to perform work on site for base access.

8.3 Contractor must provide the following information for vehicle access to the installation: pictured ID, vehicle registration and proof of vehicle insurance.

8.4 The Contractor must make every attempt to protect all government and non-government property and personnel from damage/harm. The Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the government.

8.5 The Contractor must immediately notify the Contracting Officer and secure any documents found with classified, “For Official use Only” (FOUO) or personal information. The Contracting Officer will direct disposition of these documents.

8.6 A one-year warranty on materials and labor for this project is required to assure good quality workmanship and materials, and to provide necessary maintenance and correction of deficiencies during the year following contract completion. The Contractor will provide to the Government any commercial warranty on materials or equipment included in the construction that exceeds this one-year, all-inclusive warranty.

8.7 Contractor must protect all storm water drains from any debris/sediment/chemical entrance.

Contractor must adhere to the CAFB SWPPP, to include housekeeping practices, best management practices, etc.

8.8 Contractor shall insure the follow are implemented throughout the entire project:

8.8.1 Contractor is responsible for drafting a compliance plan to adhere to the CAFB SWPPP.

8.8.2 Contractor is responsible for ensuring all trucks hauling debris/media are covered to prevent blow-off.

8.8.3 Contractor must maintain compliance with the Clean Water Act and Columbus AFB's Storm Water Permit.

8.8.4 Contractor must implement and maintain sediment and erosion controls (where needed).

8.9 Contractor must ensure silt fence is placed around areas to receive earth work prior to any earth work being performed.

8.10 Housekeeping practices must be maintained to included: proper storage, use and disposal of materials (i.e., petroleum projects, paints/solvents, acid, and detergents), Sanitary waste management (i.e., portable toilets), and trash/debris containment (i.e., prevent entry into nearby waterways or storm drains).

8.11 Provide adequate lighting for safe work area and personnel noise protection as required while engaged in work. There may be hazardous materials associated with this project. Contractor must install proper construction markings and tape during work in progress. Contractor shall maintain a hazard free work area during daily operations.

8.12 Contractor must provide notice to the Contracting Officer prior to starting any work under this project.

8.13 The Contractor must dispose of ANY and ALL debris generated from the work provided to CAFB. All debris disposal costs must be paid by the contractor. Recycle as much as practicable.

Dispose trash and debris to sanitary landfill or other means. Conform to local, state, and federal regulations. Securely cover refuse trucks before leaving the work site. If trash or debris falls from a truck, stop and reload the truck to prevent falling trash or debris. The Contractor bears liability for damage or injury resulting from falling trash or debris. Submit to the BCE-CM certified dump tickets for rubbish disposal indicating the project number. The Contractor is encouraged to contact Mike Blythe, CEIEC (434-7353) or the CAFB Recycling Center (434-7406) for on-base recycling.

8.14 The Contractor must maintain the grounds within the construction area until project completion.

ALL grounds must be returned in the condition it was given to the Contractor before construction began. The grounds shall be free of debris, leveled, and Bermuda grass used to cover any bare areas before it will be returned to the Grounds Contractor. The area should be inspected by both the Grounds Contract Monitor along with the Grounds Project Manager before the project is approved.

8.15 All material lab and testing work shall be performed by an approved commercial testing laboratory.

9. CONSTRUCTION PLAN

9.1 Construction will occur in the LRS area at the intersection of Bradt St with Sixth St and Seventh St, the rear LRS Parking lot, and the delivery and docking pavement areas – Reference Attachment.

9.2 Project will require 3 construction phases. Phase 1 includes half of the new concrete loading dock and the trenched conduit. Phase 2 is remaining concrete loading dock. Phase 3 will be remainder of asphalt and sealer work. Access to one loading dock must remain active at all times. This facility receives frequent deliveries with forklifts loading/unloading in the area. Curbing work can occur at Contractor’s discretion.

9.3 In front of the two loading docks, the asphalt pavement shall be removed, excavate existing base, compact existing subbase, place 6” crushed limestone base, and place 6” 4500 psi concrete with mesh reinforcement. New concrete will have joints with joint seal evenly spaced no more than 12.5 feet apart. See included attachments for concrete pavement area. The new concrete area will include replacing the existing concrete dumpster approach.

9.4 The new concrete pavement elevation profile must connect to the existing stormwater drainage to facilitate water runoff and prevent water ponding in areas. Only gentle sloping is permitted. This area is used primarily by forklifts carrying heavy loads.

9.5 Area must be surveyed to provide positive drainage. Contractor is to provide an elevation profile for the entire project, a detailed elevation profile of the open area in front of the LRS building, and a detailed jointing plan for the concrete area.

9.6 Demo existing concrete rolling curb gutter up to 1ft from the fence line. This area will be replaced with asphalt pavement. This location will need to be graded and prepared for asphalt pavement. See attachment for location (image 5).

9.7 Provide and install 1ea. 4” PVC sch 80 from B158 to B155. Stub up each end with a sweeping 90* elbow and cap 2ft above grade. Provide a 1800lb pull string (mule tape) tied on each end. Ditch shall be 36” deep and approx. 150ft in length. Contractor is responsible for backfilling ditch and compaction of fill materials with appropriate compaction tools. Provide a minimum of 12" cover of clean fill above the conduit. Backfill material shall be clean and free from all organic material, clay, marl or unstable materials, debris, lumps, or broken paving. No rocks or stones larger than 3 inches in diameter shall be allowed in backfill. Material for backfill may be material resulting from excavation if it meets the above requirements. See attachment for location.

9.7.1 Replace vegetation, landscaping, and any damaged curb sections in the grass area beside B158.

9.8 The Contractor shall remove the designated parking space curb and parking bumper signs prior to construction, store, and replace after pavement work has been completed.

9.9 All concrete parking bumpers are to be removed prior to construction and replaced with new bumpers after pavement work has been completed. See included attachments for location of concrete bumpers to be replaced. Total of 18 bumpers to be replaced/installed.

9.10 All existing pavement markings shall be accurately documented and annotated prior to milling existing pavement.

9.11 The Contractor shall mill 2” and overlay 2” of asphalt pavement within the LRS area, including Sixth St, and Seventh St as indicated in attachments.

9.12 Provide Tack Coat.

9.13 For areas of mill and overlay, existing asphalt must be cut 1ft from fencing and FSS RV parking gate. These sections of asphalt must be slurry sealed up to the obstacle (chain link fence and gate).

These areas are identified in the attachments with included images of areas.

9.14 Clean cracks, prepare area, and provide new sealer to LRS parking lot and repaint pavement markings in this area.

9.15 Install a concrete curb in the area between B151 and B155 as indicated in the attachments. This area was demolished in a previous project. The new curbing will need to be installed connecting to the existing curb in this area. The gravel area will need to be graded and prepared for asphalt pavement.

9.16 Replace two areas of damaged concrete curbing near the loading docks. Specific areas are identified in the attachment with included pictures.

9.17 Demolish single curb in front of B151 gate. See attachment with picture of this section.

9.18 Provide new centerline, stop bar, and vehicle parking and crosshatching to match existing with reflective thermoplastic pavement markings. Include restriping loading dock lanes in the multi-truck bay. All centerline markings shall be 6” wide.

9.19 Provide new area of crosshatching in front of gate entrances in front of LRS building. See included attachments for location reference.

9.20 Provide new blue reflective fire hydrant markers on the centerline of the roadway at fire hydrant locations.

9.21 Provide traffic control items and traffic control plan. Any traffic barricades and/or barrels must be equipped with warning lights.

9.22 All stormwater drains in the area must be maintained per SWPPP and best practices during construction to prevent outside containments from entering drainage system.

9.23 All material lab testing work shall be performed by an approved commercial testing laboratory.

9.24 Access to base facilities shall be established and maintained at all times throughout the duration of the project.

9.25 Any damage that occurs to existing pavement, vegetation, or equipment as result of contractor’s operations shall be repaired at the contractor’s expense.

9.26 The Contractor must recycle to the limit practicable locally and report all recycled materials to

QRP.

9.27 At the conclusion of the project the Contractor will be responsible to have complied with and furnish to the CO, a DD Form 1354, “TRANSFER AND ACCEPTANCE OF MILITARY REAL PROPERTY”, indicating all costs and quantity data of materials and systems furnished and installed.

A draft DD 1354 showing a list of items for which the costs and quantity data is required will be furnished to the Contractor. The completed DD 1354 will be turned into the CVO on the date of final inspection. The DD Form 1354 itself will be provided by the CO.

10. INSTALLATION/WORK PLAN

Provide adequate lighting for safe work area and personnel noise protection as required while engaged in work. There may be hazardous materials associated with this project. Contractor shall install proper construction markings and tape during work in progress. Contractor shall maintain a hazard free work area during daily operations. Contractor shall provide notice to the Contracting Officer prior to starting any work under this project. Contractor shall maintain red-line drawings of all field changes throughout construction and provide As-Built drawings of final installation prior to final acceptance by the Government.
