1.0 INTRODUCTION / BACKGROUND

The Haskell Indian Health Center was originally constructed in 1979. The entry door to the Patient Restroom for specimen collection is located inside the Lab Draw room. The Patient Restroom door should be relocated to the corridor. The wall should be closed in where the existing Patient Restroom door is removed, and a collection transfer window shall be added in this area.

2.0 OBJECTIVE

The objective of this project is to move the Patient Specimen Collection Restroom door outside of the Lab Draw area and into the corridor. Contractor shall utilize the existing door for the relocation. The area where the existing door is removed shall be walled back in and a specimen passthrough collection window/shelf shall be installed.

3.0 SCOPE OF WORK

The contractor shall remove the existing Patient Specimen Restroom pass-through door currently located in the Lab Draw room. Contractor shall reuse and relocate the existing door in the corridor and install it in a new jamb with new hardware. The new location can be seen in the attached pictures and drawings in Appendix A.

Contractor shall frame in and finish the wall area where the existing door is removed on both sides and in a manner that keeps the wall covering/theme as close to the existing as possible. Contractor shall install base/cove moldings that are in line with surrounding areas. Contractor shall utilize the upper half of this space to install a pass-through specimen cabinet between the Patient Restroom and Lab Draw Area. An example can be seen in Appendix A.

The new door opening in the corridor will require cutting into the existing CMU (Concrete Masonry Unit) block wall and adding new structure to support the new door jamb and door. Framing and appropriate header shall be of metal and ensure structural integrity.

Contractor shall install a threshold at the base of the new door opening that ensures a smooth transition between the corridor and Patient Restroom.

Contractor is responsible for performing a ICRA (Infection Control Risk Assessment) and installing safety hazard mitigation. Contractor shall implement appropriate construction protection practices considering this is a medical facility. This includes but is not limited to the installation of zipper walls and poly sheeting from floor to ceiling to limit contamination to surrounding areas.

Contractor shall supply all equipment, parts, labor, etc. to relocate Patient Restroom door, construct walls, add pass through specimen cabinet, add framework, etc.
