STATEMENT OF WORK

A. Contract Title. Controlled Substance Removal MVAMC

B. Background. This contract would provide controlled substance containers and removal with monthly service for the Minneapolis VAMC. There are three important objectives for this process:

1) Diversion (misuse) Prevention, 2) Proper Disposal, and 3) Turn-Key Removal/Replacement by the Contractor. Per MVAWC policy, the process must allow for clearly observable, witnessed destruction of waste-controlled substances. Additionally, the process must render the controlled substances secure and unusable.

C. Period of performance: The base year is April 1, 2025, through March 31, 2026 with four subsequent Option Years.

D. Scope. The intent of this contract is to provide turn-key controlled substance service for containment, removal, and disposal for Minneapolis VA Medical Centers.

1. Provide and mount up to 64 one-way containers with DEA approved anti-diversion additive chemical deactivating agent at the Minneapolis VA Medical Centers. Provide full service including exchange of filled containers, removal of waste, and ultimate destruction of controlled substance waste.

2. A professionally trained service specialist shall exchange the controlled substance containers monthly at the Minneapolis VA Medical Center. At no time, will the contractor allow containers to remain if they are equal to or more than ¾ full. A schedule of service dates and times for both facilities shall be provided to the COR prior to commencement of services. Any changes to this schedule must be provided to the COR in advance of the change.

3. Contractor selected shall not commingle services from any other contract they have with this facility with this contract. Every contracted service is separate and needs to be done separately and not commingled. The technician would sign out from other contracted services and sign in the log for controlled substances duties, then sign out for the day.

4. Contractor shall provide and install locking brackets for all controlled substance containers at the Minneapolis VA Medical Center. Installation shall be completed in one week. The contractor shall provide a total of 64 controlled substance disposal containers (25 large – 3 gallon size and 39 small – 1 gallon size) each of which are appropriate for disposal of both solids (pills and patches) and liquids. The contractor will install the brackets and respective containers which will be securely mounted to the wall, as near the existing Becton Dickson Pyxis, or like, medication dispensing machine as practicable using appropriate hardware. The containers will have one-way, tamper resistant openings at a level which allows easy insertion of controlled substances and clear observance by another employee to meet the ‘witnessed destruction’ policy requirement of these facilities. Once the controlled substance is inserted a DEA approved anti-diversion additive chemical will be used to render the controlled substance unusable. The contractor will replace the chemical cartridges when the containers are removed. Instruction guide, instructions or posters shall be provided on or near containers

5. Contractor is required to recommend and adjust container, sizes, locations, and pick-ups to meet the unique and changing requirements of each clinical area where they are to be used. Any increase or decrease to the number of containers shall be approved in writing by the Contracting Officer to ensure funding levels remain appropriate. The contract holder will remain the sole provider of this service for the Minneapolis VA Medical Center for the life of the contract and must adjust container quantities as requested by the VA.

6. Once removed by the contactor from their respective locations the containers shall be prepared and ready for removal from the facility in a timely and secure manner. Shipping is an acceptable means of removal.

7. Contractor must provide and prepare proper shipping paperwork and a log of all pickups, including a physical description of containers that are removed from each site, this log will be kept with the sign in sheets.

8. Contractor must provide Safety Data Sheets (SDS) for any chemicals it uses in the facility.

9. Performance must comply with Joint Commission, EPA, DEA, NIOSH, MPCA, MDH, MDOT, and all other applicable regulatory agencies.

10. Contractor to provide VA staff with any necessary training at installation and copies of appropriate corporate training materials.

11. The current need has been determined to be a total of 64 controlled substance disposal containers at the Minneapolis VA Medical Centers. The locations, size and container type are listed below. The 39 small containers will be 1 gallon in size, the 25 large containers will be 3 gallons in size. The Pyxis name and room number identify specific locations at the Minneapolis VA hospital. It will be the responsibility of the vendor to install the container brackets securely and neatly at specific locations to be identified by and under the supervision and assistance of these VA facilities staff, without excess disruption to patients, visitors and staff.

12. The monthly fee includes the following:

a. Initial purchase of containers and ongoing supply throughout the term of the agreement

b. Training during program rollout and training material for staff to reference throughout the life of the contract.

c. Technician who will proactively manage the collection/labeling/replacing of containers and preparing the waste for shipment

d. The monthly fee includes the cost of disposal.
