STATEMENT OF WORK

Cafeteria VCT floor removal and Epoxy floor installation

PART 1: GENERAL

1.0 PROJECT DESCRIPTION:

The contractor shall furnish all supervision, labor, transportation, materials, tools, equipment sand quality assurance to remove VCT flooring and install EPOXY flooring in the cafeteria at Sherman Indian High School (SIHS). The contractor shall provide barriers and signs to prevent unauthorized personnel from entering the work area while the contractor is refinishing the floors.

2.0 PROJECT SCOPE OF WORK:

To remove VCT flooring and install EPOXY flooring in cafeteria main dinning area, kitchen area and dish washing area at Sherman Indian High School (SIHS).

For the following building:

Cafeteria Building 9

3.0 LOCATION:

Sherman Indiah High School 9010 Magnolia Ave.

Riverside, CA 92503

4.0 CONTRACTOR REQUIREMENTS:

Technical Requirements/Tasks Task A: Rooms: Freezer, Kitchen (includes ramp extending to dining area) Manufacturer: Key Resin / Flow Resin System: Flowfresh SRQ 1/4” Cementitious Epoxy Floor System Approx. Square Footage: 1,703 SF of floor and 420 LF of 6” integral base Warranty: 12 months Approximate Duration: 10 days (curing time is an additional 72 hours) Installation Method:

· Grind existing tile and patch as needed

· Install 6” integral epoxy base up the wall over tile

· Install SRQ floor over prepped tile

· Install HF epoxy in high heat areas under vats

· Install epoxy grout coat

· Install epoxy seal coat

Task B: Rooms: Dishwashing Area Manufacturer: Key Resin / Flow Resin System: Flowfresh SRQ 1/4” Cementitious Epoxy Floor System Approx. Square Footage: 340 SF of floor and 113 LF of 6” integral base Warranty: 12 months Approximate Duration: 5 days (curing time is an additional 72 hours) Installation Method:

· Grind existing tile and patch as needed

· Install 6” integral epoxy base up the wall over tile

· Install SRQ floor over prepped tile

· Install epoxy grout coat

· Install epoxy seal coat

Task C: Rooms: Washroom, Food Storage, Janitor, Boys, Girls Manufacturer: Key Resin / Flow Resin System: Flowfresh SRQ 1/4” Cementitious Epoxy Floor System Approx. Square Footage: 850 SF of floor and 268 LF of 6” integral base Warranty: 12 months Approximate Duration: 7 days (curing time is an additional 72 hours) Installation Method:

· Remove existing vct flooring and rubber base

1. Remove all thin set mortar and all other setting materials

2. Repair existing underlayment

3. Dispose of all materials properly off site.

· Grind existing slab and patch as needed

· Install 6” integral epoxy base up the wall

· Install SRQ floor

· Install epoxy grout coat

· Install epoxy seal coat

Task D: Rooms: Kitchen, Freezer, Dishwashing, Washroom, Food Storage, Janitor, Boys, Girls Manufacturer: Key Resin / Flow Resin System: Flowfresh SRQ 1/4” Cementitious Epoxy Floor System Approx. Square Footage: 2,892 SF of floor and 802 LF of 6” integral base Warranty: 12 months Mobilization(s): 1 Approximate Duration: 20 days (curing time is an additional 72 hours) Installation Method for Epoxy:

· Remove existing vct flooring and rubber base

1. Remove all thin set mortar and all other setting materials

2. Repair existing underlayment

3. Dispose of all materials properly off site.

· Grind existing tile and patch as needed

· Grind existing slab and patch as needed

· Install 6” integral epoxy base up the wall

· Install SRQ floor

· Install epoxy grout coat

· Install epoxy seal coat

Task E: Rooms: Dining, 2 Hallways Manufacturer: Convergent System: Polished Concrete Floor System Polish Finish: Light Aggregate Exposure, Gloss Level 1 – 400 Grit Finish Approx. Square Footage: 5,996 SF of floor and 374 LF of 4” rubber base Installation Method for Polish:

· Remove existing vct flooring and rubber base

1. Remove all thin set mortar and all other setting materials

2. Repair existing underlayment

3. Dispose of all materials properly off site.

· Grind existing slab and patch as needed

· Polish concrete to a Level 1 Gloss Finish

· Seal existing joints

· Install 1- coat of densifier

· Install 1- coat of guard sealer

· Install 4” rubber base throughout

6.0 HOURS OF WORK:

Normal duty hours are from 8:00 a.m. to 4:30 p.m. Monday thru Friday, excluding weekends and observed Federal Holiday.

7.0 PERIOD OF PERFORMANCE:

PART II - GENERAL REQUIREMENTS DURING CONSTRUCTION

1.1 CONTRACTOR USE OF PREMISES:

A. Sanitary Facilities: The Contractor shall provide and maintain necessary sanitary conveniences for the use of those employed on or about the work. The sanitary facilities shall be properly secluded from public observation and locations shall be approved by the Government Project Inspector. Sanitary conveniences shall be fully enclosed chemical toilets, and properly ventilated. The premises shall be kept in a neat and sanitary condition at all times. Use of the toilets within the buildings, by the Contractor and construction personnel, shall not be permitted.

B. Project Office Building: The Contractor may provide an office building at their own discretion for their use during the construction phase. None will be provided by the Government.

C. Construction Water: The Contractor shall obtain construction water from a local source or if approved by SIHS. Obtaining suitable quality and quantity of construction water is the responsibility of the Contractor.

1.2 OCCUPANCY REQUIREMENTS:

The school will be in operation during the renovation and construction period. The Government will require site access. Cooperate with the Government during construction operation to minimize conflicts and facilitate Government usage. Perform the work so as to not interfere with the Government's operations.

1.3 CONTRACTOR CONFERENCES AND MEETINGS:

A. Pre-Construction Meeting: After the project is awarded and before the Notice to Proceed is issued, a Pre-Construction Meeting will be conducted.

B. Scheduled Construction Meetings: Unless determined otherwise by the Contracting Officer, site construction meetings will be conducted weekly for the contractor and his subcontractors. The purpose of the meeting is to update the work performance schedule and discuss safety awareness issues on the job. Monthly progress meetings will be held at the site to update the CO and COR on the project progress.

1.4 BULLETIN BOARD:

The Contractor shall furnish and install a bulletin board to be used for the purpose of displaying wage rates, fair employment poster and similar material. The bulletin board shall be weatherproof and be posted during the duration of the project. It shall be located in a general location.

1.5 SCHEDULE OF WORK:

In order to provide continuous use of the facilities during the construction time, the Contractor is required to pursue the work in accordance with an approved work schedule. The Contractor shall prepare and submit a schedule, pursuant to FAR Clause 52.236-15, entitled, "Schedules for Construction Contracts", in which is described methods and time sequence necessary to complete the work. All work scheduling shall be subject to the approval of the Contracting Officer.

1.6 WORK CONDITIONS:

A. Precautionary Measures: The Contractor shall take precaution to protect all work during construction period and shall provide and maintain adequate protection to prevent damage to the work or to the materials. Safety measures shall include all necessary protective barriers such as fencing, barricades, covered walkways, enclosures, etc., proper posting with warning signs and/or signals, and adequate lighting. All Contractors' new hires shall be informed of Occupational Safety and Health Administration (OSHA) safety regulations at the beginning of the job.

B. Contractor's Failure to Comply with Safety Requirements:

1. It shall be Contractor's responsibility to comply and enforce all contract safety requirements for his employees. Contractor shall monitor and ensure all subcontractors are in compliance.

2. Upon notice of a safety violation, the Contracting Officer (CO), Contracting Officer's Representative (COR), or Inspector will notify Contractor's employees in the immediate effected working area of the violation and next report it to the Contractor's project superintendent. The project superintendent shall correct the safety deficiency by providing a safety abatement plan and report such incidents during the weekly safety meetings.

3. The Project Inspector shall have authority to order work be temporarily suspended in specific work location(s), should unsafe working conditions be hazardous to contractor's employees, sub-contractor(s), or public's personal life or health. All such incidents shall be reported immediately to the Contractor's project superintendent, contractor, COR and CO. Date and time of incidents is required, photographs are also preferred.

1.7 DEBRIS DISPOSAL:

The Contractor owns all debris generated by or caused by the work in this contract. The Contractor bears all responsibility and all costs connected with or related to the removal and disposal of all debris from properties of the United States Government. The Contractor is required to comply with applicable Environmental Protection Agency (EPA) regulations, Federal, State and local Government laws and regulations while engaged in the removal and disposal of debris. The Contractor shall remove the debris. The Contractor shall remove the debris off of the boundaries of the to an EPA approved landfill for the types of debris being deposited. The Government is not responsible in any way, financial or otherwise, for the success or failure of arrangements for disposal of debris between the Contractor and other parties.
