STATEMENT OF WORK

AFIA Bldg. 24499 3rd Floor Flooring Replacement

1. Scope: The contractor shall furnish the materials, labor, equipment, and supervision necessary to move furniture, remove existing carpet, cove base, and install new carpet tiles, luxury vinyl tile (LVT), and cove base in rooms 317 to 337, 341, 342, 345, 346, 347, 350 to 357, 360 to 366, 369 to 387 of Building 24499, at Kirtland Air Force Base, New Mexico. The contractor shall meet all requirements discussed in this Statement of Work (SOW).

The mandatory United States Air Force Carpet Program applies to this effort. The contractor shall procure carpet tiles and adhesive through the Tier 1 vendor. Please see Section 9 below for instructions, user choice selection, and Points of Contacts (POC) applicable to this SOW.

1.1 Furniture Movement: Movement and replacement of non-fixed furniture (i.e. chairs, desks, bookcases, storage cabinets) shall be the contractor’s responsibility. This project is to occur in conjunction with replacement of cubicles throughout the entire floor, where a separate contractor (herein referred to as the ‘furniture contractor’) will be removing semi-fixed furniture (i.e cubicle walls). The contractor shall be responsible for coordinating installation times with the furniture contractor.

1.2 Carpet Tile and Cove Base Removal: The contractor shall remove and dispose of approximately 32,500 sq ft of existing carpet and cove base.

1.3 Floor Preparations, Carpet. LVT, Cove Base Installation: The floor shall be cleaned and prepped.

New carpet tiles (approx. 31,000 sq ft), luxury vinyl tile (LVT) (approx. 1500 sq ft), and cove base shall be installed in phases shown in paragraphs 1.3.1-1.3.3 by the contractor utilizing mandatory Tier 1 vendors. See Section 9, Special Requirements. Once installed, carpet tiles and cove base material shall be level, square, and in proper alignment with adjoining material, walls, and furniture. In continuance, the new carpet tiles shall be clean, free of fraying, smooth and properly installed. The government shall inspect carpet tiles and cove bases prior to final acceptance. A commercial grade, metal style transition shall be used for all transition thresholds. The contractor shall remove existing thresholds and reinstall. The contractor shall be responsible for and shall replace thresholds if they are damaged during removal.

1.3.1 Phase A1: rooms 341, 342, 345, 346, 347, 350 to 357, 360 to 366, 369

2. Period of Performance: Duration of this task is anticipated to be from date of award through 12 weeks.

3. Weekly Updates: The contractor will provide weekly updates to the facility manager. The weekly update shall include at a minimum, the progress of the project, any delays, all requested service outages, all access requests, and any safety or damage incidents.

4. Performance Locations: Performance location includes the third floor of Building 24499. At the end of each shift, the contractor shall ensure all debris and surplus materials from the workplace are removed.

Safety barriers shall be in place to protect unfinished work site at the end of the day. Equipment and materials related to this project may remain on site; however, they shall be organized, stored in a manner that does not create a safety hazard, and approval from the Customer POC has been granted. The Government will not be responsible for the loss, theft, or damage of equipment or material stored on-site.

5. Warranty: The Contractor shall provide a one-year written warranty or manufacture’s standard commercial warranty, whichever is longer, to cover any deficiencies and failures due to poor workmanship and/or material defects. Tier 1 Vendors provide warranties for the products they manufacture against defects of material and workmanship under normal use as well as service from date of install. The Contractor shall contact the Tier 1 Vendor for specific information regarding all standard commercial warranties. The Contractor shall also correct any deficiencies and damages caused by the installation Contractor at the installer’s own expense. The contractor shall provide written procedures and required information for warranty services at or prior to site acceptance.

6. Scheduling: The Contractor shall provide an accurate work plan (schedule) within 10 business days of contract award. The plan shall be coordinated with both the Customer POC and the Furniture Contractor.

The schedule shall include number of days to complete as well as a detailed functional plan indicating when tasks described in this SOW will begin and end. The Contractor shall also coordinate with the Contracting Officer to start work and maintain communications throughout the life of the project.

7. Security Requirements: Stated work and associated products shall be performed at the UNCLASSIFIED level. Escorts are the responsibility of the government and will be provided in accordance with the schedule provided by the Contractor.

8. Safety Requirements: The Contractor shall remain in compliance with all Federal, State, local and Kirtland AFB security and safety laws, regulations, policies, and requirements.

9. Special Requirements

9.1 The United States Air Force Carpet Program: The mandatory United States Air Force Carpet Program applies to this effort. The Contractor shall procure carpet tiles and adhesives through a Tier 1 Vendor. See Table 1, Tier 1 Vendors below.

9.2 Materials: The Government has identified a total of seven (7) suitable carpet selections and seven (7) suitable adhesives: one from each Tier 1 Vendor. For more information see Table 2, Materials. 10. Hours of Operation: The Contractor shall be responsible for conducting business between the hours of 0730-1600 Monday thru Friday, except federal holidays and when the Government facility is closed. This may include but is not limited to local and/or national emergencies, administrative closings, and/or similar Government directed facility closings.

11. Base Access: The Contractor shall ensure all commercial vehicles within their purview enter through the Contractor’s gate located just before the intersection of Gibson Blvd. and Carlisle. Base access procedures and delivery vehicle process requirements mandated by Kirtland Air Force Base shall be adhered to by the Contractor. The Contractor shall coordinate with the Contracting Officer to obtain access to the base. For current gate hours and closures, the Contractor may download the Kirtland Air Force Base mobile application via the App Store by searching “KAFB” or “Kirtland Air Force Base” in your phone’s App Store. The Contractor may also access this information via the Kirtland Air Force Base webpage at https://www.kirtland.af.mil/. Once at the gate, drivers must be able to show/provide:

• Current vehicle registration

• Current Proof of Insurance

• A valid and current Driver’s License for the driver; current base access requires contractors be REAL

ID compliant

• A valid and current Driver’s License or other State Identification for any passenger(s) in the vehicle;

current base access requires contractors be REAL ID compliant

• A Bill of Lading which includes the Air Force installation name, date of anticipated delivery, and specific destination information

12. Federal Holidays: Contractor shall refer to the OPM link below to view all current federal holidays, for which Government offices will be closed.

13. Post Award Conference: A date, time, and meeting location will be provided to all parties upon contract award.

14. Trash/Waste Removal: The contractor is responsible for all waste management related to this effort. The contractor shall remove all trash, waste, and/or debris from the worksite daily. The contractor shall transport and properly dispose of all trash, waste and/or debris off-base. Packaging and other recyclable materials shall be recycled in an environmentally designated area off-base, and trash/waste shall be disposed of in accordance with all local and state laws. A dumpster may be brought to the facility for debris disposal at contractor expense; however, the contractor shall obtain facility manager approval prior to coordinate where it shall be located/positioned.

15. Facility Protections: During installation, the contractor shall protect all furniture and building materials within the space, utilizing industry approved protective coverings inclusive to craft paper, moving blankets, and other such protective materials. The contractor shall not use protective material which may leave any type of residual print, outline, or imaging on any furniture or building materials.

16. Shipping Damage: The contractor shall inspect all product for shipping damage. If damage is found, it shall be the responsibility of the contractor to notify the authorized Government Representative and make all arrangements for replacement or repair of the item(s) damaged. Damaged items are not to be installed unless otherwise directed by the contracting Officer.

17. Damage to Facility: The contractor and their representatives shall be liable for any damage they cause to the facility or any Government property. If any damage were to occur, the contractor or their representatives shall not perform any repairs themselves, but they must hire a licensed General Contractor to perform the work at no cost to the Government.
