WIND RIVER AGENCY
GARAGE DOORS PROJECT
A) BACKGROUND
Statement of Work
1) The Branch of Facilities Management and Construction will provide construction funding for Garage Door replacement at Building 123 and 132. This will include removal, disposal and replacement of 16 Garage
Doors at the Wind River Agency at Fort Washakie, Wyoming.
B) OBJECTIVES
1) The Bureau of Indian Affairs (BIA), through a contract, will provide for the removal and disposal of existing Garage Doors and the installation of 11 new Garage Doors w/ ½ hp motors and operators for buildings 123 and 5 new Garage Doors w/ ½ hp motors and operators at building 132 at the Wind River
Agency. The construction work shall be based on the approved proposal from the selected vendor. All construction work will adhere to applicable federal, state, and local codes, including approved drawings/plans and specifications. The objective of this Statement of Work (SOW) is to define and establish a general scope of services necessary for the Garage Door removal and installation.
C) SCOPE
1) This contract includes but not limited to the removal and disposal of existing Garage Door materials and the installation of a new Garage Doors W / ½ hp motors and remote operators andJill associated Garage
Door materials for each Garage Door at the agency buildings listed at the Wind River Agency at Fort
Washakie, Wyoming. This contract is for Labor, equipment, materials and the oversight to complete the project in accordance with manufacturer's recommendations. The contractor shall be responsible for providing all costs associated for the replacement of all Garage Doors, materials, clearances, and placing of all existing Garage Door motors in a secured location, all accordance with all federal and state regulations. The contractor shall be responsible for storage, final shipping and delivery cost from the factory to the Garage Door materials final site. The Contractor is recommended to visit the site to perform field measurement and identify work items so the proposal submitted will be complete. The
Contractor shall verify the actual quantity removed by the Contractor in preparation for the Garage Door replacement by the General Contractor. Plans developed under this SOW will be used as final construction documents for new Garage Doors. The scope of work includes but not limited to:
a) Detailed Contract Price Schedule
b) Detailed Construction Cost Estimate
c) Shop Drawings (Attachment).
d) Closeout and Operation & Maintenance (O&M) Requirements
2} The contractor shall provide all Labor, equipment and materials to complete the project in accordance with the attached plans and specifications.
3} All existing Garage Doors and material shall be removed and disposed of according to all Local, State and
Federal regulations. Motors for the Garage Doors shall be salvaged for future reuse on other doors, they will be stored in secure location designated by the Agency Facility Manager.
D) TASKS
1) The Contractor will provide performance bonding and professional and contracted services for the construction of the new Garage Doors.
2) The Contractor shall provide Garage Door material for all listed Agency buildings in compliance with project plans, specifications and supplemental documentation.
3} These quantities are for informational purposes only and are based on the best information available at the time of the specification preparation. The Garage Door Contractor shall verify the actual quantities

J) Other Unique Requirements or Considerations
1) A site visit will be conducted 10 days after issuance of solicitation, all Vendor's are recommended to attend. Location and Time will be scheduled 10 days after solicitation.
2) All structures and areas will be occupied during this project.
3} The Contractor shall contact BIA COR, at the phone number indicated above, for questions or discussions regarding technical matters relative to the services required. This individual will be designated as the COR for the contract. The Contractor will contact the CO, at the phone number indicated above, on all other matters.
4) Any changes to the Program of Requirements which create a change in the building design or construction schedule or cost shall be submitted to the COR by the Contractor, for review and to the CO for approval.
5) The BIA will review and approve the Contractors Construction Schedule and Schedule of Values and allow the BIA CO and COR to review the proposed schedules.
6} The BIA will review and approve Contractor's Quality Assurance Plans and provide a copy of the plan to the BIA and COR for information, review and comment.
7) The BIA will review and approve construction Contractor's Accident Prevention Plan and provide a copy of the plan to BIA's COR for information.
8) The BIA will participate in weekly meetings, the final inspection(s) and the punch list inspection(s).
9) The BIA's COR will also make periodic site visits at critical times during the construction process, however the BIA's COR shall not direct the Contractor.
10) The Contractor, of their representatives sh al I not be allowed to make changes in the work. Only the BIA
CO is authorized to make changes to the funding or scope of work in the construction contract.
11) Egress traffic, street access, building access and BIA employee parking shall not be encroached upon.
K) Place of Performance
1) Department of the Interior, Bureau of Indian Affairs, Wind River Agency, Fort Washakie, Wyoming 82514
L) Period of Performance
1) Acceptable working hours are 07:30 a.m. to 5:30 p.m. Monday through Friday. Working weekends will be by request only and approved by the Agency Facilities Manager.
2) This project shall be completed forty (90) calendar days after receipt of performance and payment bonds.
3) liquidated damages shall be assessed on 91st day after receipt of Performance and Payments Bonds. If the project is not completed liqudatedamages shall continue, at the rate of $506.32 per day until COR confirms the project is completed.
