QUALITY ASSURANCE SURVEILLANCE PLAN

QUALITY ASSURANCE SURVEILLANCE PLAN

Performance Standard Monitoring Method Acceptable Quality Disincentive Exposure facilities are available & fully functional when scheduled subjects arrive; no exposures are cancelled for lack of facilities.

The Contractor shall report and explain instances when exposure facilities are not available or operational; research investigators will also informally report research failures.

No more than one subject cancellation per exposure system per year

Exceeding the one cancellation per system allowance will result in a payment reduction of 1% of the fixed price portion of the next monthly payment.

Emergencies involving breakdown or loss of performance of exposure systems or research support systems, creating a situation where planned research cannot be performed, shall be evaluated within 2 hours and repaired or an acceptable plan developed within 48 hours.

Occurrence of a single unaddressed emergency is unacceptable; for each year, one instance of failing to develop a repair plan in a timely manner will be accepted.

Maintenance and inspection schedules will be developed and implemented for all equipment assigned to the contractor.

develop a 12 month schedule for inspect-ing and maintaining all equipment, dividing the schedule into monthly increments and reporting on these in monthly progress reports.

All equipment shall be inspected and maintenance performed within 30 days of established due dates.

Exceeding the stated limits for 5 or more items for a total of more than 3 months will be reflected as less than satisfactory performance in the past performance report card.

Special projects, upgrades and new facilities are completed, tested and available in a timely manner.

With PO concurrence, the Contractor shall develop a schedule for completing & testing the project and report on progress in the monthly report.

New projects, upgrades and facilities should be available within 30 days of their estimated completion dates.

Exceeding due dates by 30 days or more will result in a payment reduction of 1% of the fixed price portion of the next monthly payment.
