About this file
This is an Offeror Self-Performance & Subcontracting Disclosure Checklist template for compliance with FAR 52.219-14, which addresses limitations on subcontracting for federal contracts.
The checklist requires offerors to complete all sections or face potential ineligibility for award. Section 1 asks whether the prime contractor will self-perform the required work; if yes, the offeror skips to Section 5. Section 2 requires identification of subcontracting intent. Section 3 mandates disclosure of projected subcontractor information for each anticipated subcontractor, including legal business name, Unique Entity ID (UEI), assigned NAICS code, small business size representation under that NAICS code, and small business program status (Small Business, SBA-certified SDVOSB, WOSB/EDWOSB, HUBZone, or 8(a)). Section 4 requires the prime contractor to certify compliance through three checkboxes: that subcontracting will not exceed the allowable percentage to non-similarly situated firms, that all listed subcontractors have provided NAICS-specific and program-status representations with reasonable reliance basis, and acknowledgment that FAR 52.219-14 non-compliance may result in termination for default, negative CPARS ratings, or other government remedies. The certification must be signed by an authorized official with printed name, title, and date. Section 5 provides optional space for clarifying remarks regarding performance approach or subcontracting strategy.
