About this file
This is a Solicitation Survey form (Solicitation No. W912WJ26QA172) that vendors must complete to be compliant with the solicitation requirements. The form emphasizes that all information must be provided or quotes will be deemed non-responsive, and vendor information must match SAM registration assertions to remain acceptable for consideration.
The survey requires vendors to provide detailed company information including name, CAGE code, address, and point of contact. Vendors must document their years of experience in the services, contracting history as prime or subcontractor, and Federal Government contract experience. The form requires disclosure of any failed work completion with specified reasons, and vendors must declare whether they have experienced terminations. For key vendor personnel, vendors must specify minimum and maximum employee counts, confirm regular payroll status, and identify whether additional hiring is needed. Vendors must list current position/title and years of experience for each employee performing the work. The survey requires vendors to provide certifications or licenses in First Aid/CPR, Competent Person in Fall Protection, Rope Access, applicable trade licenses (electrician, plumber, etc.), Crane Operator/Qualified Riggers, and Boat Operator status; if certifications are incomplete, vendors must provide detailed plans including course names, providers, training dates, and course links. For subcontracted work, vendors must provide subcontractor name, point of contact, and location (with "to be determined" responses resulting in non-responsive determinations), or enter "NONE" for self-performing work. Teaming arrangements with subcontractors must be documented with signed confirmation or vendors may receive lower ratings. Past performance references must be completed within the past three years and include contract agency/company name, contract number, total value, complete POC information, contract dates, and sufficient project scope details; government contracts are preferred, and subcontractor past performance is only considered with suitable teaming arrangements. The form concludes with a certification section requiring the certifying official's name, title, and date, affirming the accuracy and completeness of all statements and authorizing reference contacts.
