About this file
This is a Solicitation Survey form (W912WJ26QA167) required as a compliance document for vendors submitting quotes to the U.S. Army Corps of Engineers. All information must be provided in full; failure to complete the survey results in quotes being deemed non-responsive. The survey collects company and personnel information exclusively from the prime contractor submitting the quote, with subcontractor information permitted only when a suitable teaming arrangement is documented.
The survey requires vendors to provide: years of experience in the services and federal contracting; disclosure of any failed work contracts with government contact authority for verification; key personnel availability with minimum and maximum employee counts and payroll status; detailed employee experience information for all personnel performing work; certifications and licenses in First Aid/CPR, Fall Protection Competency, Rope Access, applicable trade licenses, Crane Operation/Rigging, and Boat Operation, with plans for obtaining missing certifications including course names, providers, dates, and links; complete subcontractor information including name, point of contact, and location if applicable, or "NONE" if self-performing; and three past performance references completed within the past three years showing preference for government contracts with sufficient detail including contract value, agency/company name, contract number, POC information, start/end dates, and detailed scope descriptions. All information must be substantiated by SAM registration assertions, and vendors with unsubstantiated claims or terminations on record are subject to removal from consideration. A certification statement signed by an authorized official with date is required at the conclusion of the survey.
