About this file
This is a Solicitation Survey form (Solicitation No. W912WJ26QA154) that vendors must complete in their entirety to be deemed compliant and responsive to this federal contract opportunity.
The survey requires vendors to provide comprehensive company information including name, SAM CAGE Code, address, point of contact details, and business type classification. Vendors must document years of experience in the required services, years as a prime or subcontractor, and years performing federal government contracts. The form mandates disclosure of any work failures, with false "No" responses resulting in removal from consideration if terminations are discovered. Vendors must identify all key personnel (excluding subcontractors) who will perform work, specify minimum and maximum employee counts, confirm regular payroll status, and indicate hiring needs. The survey requires detailed employee experience information for all performing personnel and mandates submission of specific certifications or documented plans for obtaining them, including First Aid/CPR, Competent Person in Fall Protection, Rope Access, Crane Operator/Qualified Riggers, Boat Operator, Certified Industrial Hygienist, and State of Connecticut Asbestos Removal Certification. For vendors utilizing subcontractors, complete subcontractor identification with name, point of contact, and location must be provided at submission; vague responses render quotes non-responsive. Past performance experience must be documented with three contract references completed within the past three years, including contract details, agency/company name, contract value, complete POC information, dates, and detailed scope descriptions. Government contract experience is preferred but not required. A certification statement must be signed by an authorized official attesting to the accuracy and completeness of all submitted information.
