Description/Specifications/Statement of Work

STATEMENT OF WORK

What the Department of Optometry Needs:

- Multiple scleral lenes with diameters that fall in the range of 14 mm to 20 mm for a large range of corneal sizes.

- The ability to have a prolate or oblate scleral lenes design to fit a wide range of corneal shapes.

- The ability to have custom design options to fine-tune optics and fit.

- The ability to talk with fitting consultants regarding specialty contact lens fit.

Scleral lenses are used to treat irregular corneas and ocular surface disease. Scleral lens often allows patients with compromised corneas and decreased vision to have surgery postponed or canceled due to the high quality of vision scleral lenses can provide. Scleral lenes are filled with preservative free fluid which the anterior surface of the eye continuously bathes in. The fluid in the bowl of the lens creates a smooth optical surface which corrects corneal irregularities and ultimately improves a patient's visual acuity and symptoms related to ocular surface disease.

The Department of Optometry sees many patients at WAMC with compromised vision and are referred toby other providers within our system. Scleral lenses are highly specialized lenses, which involve an average of ten specifications per lens, and often require microscopic adjustments that are the thickness of a human hair.

Contractor should be able to provide lenses within ten days. They shall provide warranties allowing changes to be made to the lenses at no cost to the ordering facility.

For contractors that do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative , or, at OCONUS locations, in accordance with status of forces agreements and other theater ) regulations.

For contracts that require OPSEC Training. Per DHA-AI 5200.03, Operations Security, the contractor employees must complete Level I OPSEC Awareness training. New employees must be trained within 30 calendar days of their reporting for duty and annually thereafter
