About this file

This is a Questions and Answers document addressing clarifications for a contact lens supply solicitation with an associated Blanket Purchase Agreement (BPA).

The solicitation seeks pricing for all contact lens types, including scleral lenses with base lenses, custom designs, optical options, coatings, and modifications. Quoters may submit a manufacturer's complete catalog with separate pricing for each category. The BPA structure permits a nonclinical prime contractor to serve as the ordering and billing entity while WAMC's licensed optometrists provide prescriptions and fitting specifications directly to the manufacturer, with all clinical decisions remaining with WAMC providers. Delivery must occur within 10 business days from receipt of a completed prescription and fitting specifications. A 90-day warranty covers all changes and associated shipping at no cost. Quoted prices must remain fixed for the entire 60-month BPA period; price revision requests may only be submitted in writing at least 30 days before the proposed effective date.

Quotations must be submitted in four separate volumes: Administrative, Technical, Past Performance, and Price. The Technical volume may include the manufacturer's product literature, specifications, fitting guides, warranty information, and delivery confirmation, accompanied by the prime contractor's technical narrative. Quoters may provide relevant manufacturer or supplier past performance when that entity will manufacture and fulfill the lenses, provided the submission clearly identifies the entity whose experience is presented. The amended offer due date is 28 August 2026 at 10:00 AM Hawaii Standard Time.
