*Dual hatting will not be allowed.
For each of these required key personnel, the offeror shall identify the available capacity and any current projects they are working on with the anticipated completion dates. The Offeror shall describe its approach for managing performance under this solicitation, including techniques for management of work coordination with subcontractors and measures/corrective action that will be taken for substandard performance. Magnitude: Each submitted project must have a total value exceeding $20 million.
Scope: Projects must involve construction of a fully functional chiller plant, demolition, electrical work, site development, utilities installation, and structural and mechanical construction. Preferably projects completed within VHA facilities.
Complexity: Projects must demonstrate chiller plant construction in an active healthcare setting, with minimal system downtime. This includes effective phasing, coordination of critical path activities, and a detailed understanding of the statement of work.
Offeror shall provide a detailed summary of least two (2) and no more than three (3) recent projects similar in size, scope, and complexity. Each summary shall include:
1. Owner Government Agency/Company which awarded contract and their contact information (Name, Title, Telephone Number, Email);
2. Contract Number;
3. Location (physical address) of project performance;
4. Project Name;
5. Project Description (Details of construction requirements, challenges, and solutions);
6. Project Cost and Cost Control Measures (original and final contract costs, contract mods, cost containment measures, value engineering, etc.);
7. Project Performance Period (Initial and actual, schedule impacts and efficiencies); and
8. Subcontractors (Name, Roles, Responsibilities, Percent of work).
*May utilize RFP Attachment 7 – Project Information Sheet for Contractors to provide above information if preferred.
In addition, for each project submitted, Offeror shall provide a narrative describing successful performance in the construction of a fully functional chiller plant similar in scope to Milwaukee VAMC Project #695-24-101. The narrative shall include details on complexity, magnitude, challenges and degree of success. Offeror is encouraged to include supporting documents such as photographs. The narrative should address challenges with schedule, unforeseen site conditions, or other unusual factors, and explain how these challenges were overcome.
At least one of the projects submitted must have been completed by the Prime.
Offerors lacking relevant past performance history may submit information regarding predecessor companies or key personnel who have performed on other contracts. Such information shall, as a minimum, include: Name(s) of Predecessor Company/Sub-contractor and/or Key Personnel and include: Complete Address and Point of Contact;

For offerors that are joint ventures, the evaluation will take into account past performance of the joint venture as well as the past performance of each party to the joint venture. Offerors are cautioned that they must be registered and certified in the SBA certification database on the SBA Veteran Small Business Certification Program portal at https://search.certifications.sba.gov/ and must meet federal small business size standards for the NAICS code assigned to this solicitation at time of both proposal submission and at time of award.
Quality Assessment: If a completed past performance evaluation is available in CPARS, it shall be submitted with the proposal for each project identified. If a CPARS report is not available, a completed and signed Past Performance Questionnaire (Attachment 8 - PPQ) must be submitted in lieu of a CPARS report for the project. If an offeror has no relevant past performance history, the offeror must affirmatively state it possesses no relevant past performance history. Without this statement, the Offeror may be rated less favorably for failure to submit the required information.
Offerors without relevant past performance or for whom past performance information is not available cannot be evaluated favorably or unfavorably for this Factor. Past performance evaluations may also be conducted using information obtained from CPARS and any other sources deemed appropriate by the CO. Other sources may include, but are not limited to, inquiries of owner representative(s), Contractor Performance Assessment Reporting System (CPARS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the Offeror.
Since the Government may not necessarily interview all the sources provided by the Offerors, it is incumbent upon the Offerors to explain the relevance of the data provided.
Offerors are reminded that the burden of proving low performance risk rests with the Offerors
Evaluation Methodology: The Government will conduct a performance risk assessment based on the recency, relevance, and quality assessment of the Offeror's past performance. This assessment will determine the degree of risk associated with the Offeror's ability to successfully perform the requirements of this solicitation. The overall performance risk assessment will be comprised of ratings assigned to relevancy, quality assessment, and recency of all the projects.
If a completed past performance evaluation is available in CPARS, it shall be submitted with the proposal for each project identified. If a CPARS report is not available, a completed and signed Past Performance Questionnaire must be submitted in lieu of a CPARS report for the project.
If an offeror has no relevant past performance history, the offeror must affirmatively state it possesses no relevant past performance history. Without this statement, the Offeror may be rated less favorably for failure to submit the required information.
If a project is not recent, it will not be evaluated further meaning a relevancy rating nor performance risk rating will be assigned. Projects that are determined to be not relevant will not be assessed for quality.
Past performance demonstrating higher levels of success and quality on recent and relevant projects similar in scope and magnitude to this solicitation present lower levels of risk and will be more favorably rated. Offerors should make every attempt to provide past performance for projects cited furnishing a Past Performance Questionnaire completed by the customer or a CPARS rating, if applicable and available.
Offerors without relevant past performance or for whom past performance information is not available, the offeror cannot be evaluated favorably nor unfavorably on past performance.
