About this file
This is an Architect/Engineer (A/E) Specification and Design Checklist for the Milwaukee VA Medical Center, serving as a comprehensive guidance document for design procurement and construction project management. The checklist establishes detailed design responsibilities between the A/E firm and the Contracting Officer's Representative (COR), with separate tabs addressing design phases, specifications, drawings, required inspections, waste management, equipment sourcing, data cabling standards, and risk baseline assessments.
The checklist outlines mandatory deliverables across four design phases: Charrette Report, 35% Schematic Design, 65% Design Development, and 95/100% Construction Documents. Key requirements include coordination with users, infection control specialists, safety personnel, OI&T, facility management leadership, and specialty consultants. Critical items requiring COR identification and approval include asbestos abatement testing, bid items, equipment layouts, medical gas requirements, phasing plans, sole-source versus basis-of-design determinations, commissioning requirements, and 19 separate categories of VA-provided/approved systems. The document specifies sole-source vendors for systems including Johnson Controls (HVAC controls/PACS), Systec (fire alarm/nurse call), and established design standards for equipment such as Guldmann patient ceiling lifts, Swisslog pneumatic tubes, and Herman Miller modular furniture. Required inspections span 25 checkpoints from pre-site conditions through final inspection, with detailed specifications for utility metering, electrical panel nomenclature, data cabling (Cat 6A with LC connectors), and division-by-division specification edits. The checklist includes mandatory compliance with VA standards for interior finishes, hardware scheduling with Medeco 7-pin cores, infection control barriers, and construction waste management per LEED requirements.

AE Design Checklist VAMC Milwaukee AE Design Checklist
This design checklist and the AE SOW need to be considered together early in the design procurement phase. The intent of the checklist is to confirm AE design considerations and VA standards during design are followed. COR should review checklist during 35/65/95 Design Review meetings. This design checklist should also be sent to AE along with AE SOW and Specification Checklist during design procurement.
| Project Name: | |
| Project Manager: | Proj No: |
| Div. | Description | AE Acknowledge Initial | COR Review Initial |
| AE SOW Template is located: FM PROJECT MANAGEMENT > 14-Procurement Package Documents > 01-AE Design Procurement. |
Coordinate with Users+ to update the AE SOW while you review this DESIGN CHECKLIST.
Note: Users+ = Users, Infection Control Nurse, Safety (Fire Marshall/Construction Inspector), OI&T/Telecom, FM Leadership, Interiors Dept, GEMS, Logistics, Patient Safety, EMS, Police, BioMed, Pharmacy (PYXIS?), Shops.
PD = Pre-Design; these items need to be considered during AE SOW development and can be finalized during early design review meetings with AE.
GEN = General Design Checklist note
| PD | Equipment procured outside construction contract? High-Tech/High-Cost equipment may be procured by Biomed/Users/Logistics. Coordinate early during SOW development. |
| PD | Pyxis Supply - Power and data connections (Clean Utility Rooms). Discuss Pyxis with Users and Pharmacy. Ensure minimum requirements for power/data/space are available. |
| PD | Sole Source versus Basis of Design: COR to review material requirements and make the determination between Sole Source and Basis of Design. Review light fixtures, plubming fixtures, ceiling tile, paint, sole source list (see SN Spec) and any other material issue that may become a basis of design/sole source issue. AE SOW must inform AE what our design standards (Basis of Design and Sole Source) are. COR prepares justification documents w/ AE preparing cost of sole source items. AE shall provide list of Basis of Design. Sole Souce list in Special Notes specification will need to be edited by the COR. Sole Source justification documents are part of the Construction Procurement Package, so work with AE to get cost breakdown of each. |
| PD | Phasing and Moves shall be roughed out by SD deliverable. Will the construction and construction phasing require moves of staff? If so, begin discussing this with Users+ and ensure phasing notes allow for time for VA to move staff as necessary prior to turning over each phase to the contractor. Moves of patients and staff shall be reviewed and approved by FM Leadership and TMT. Final phasing plans and move plans shall be approved by Users and Director and incorporated in drawings. |
| PD | Review Sole Source or Design Basis tab this file - coord with FM Leadership, Users+ to make correct plans for equipment that needs to be sole sourced or made basis of design (with allowable equal). There will be spec work as needed. |
| PD | Ensure AE has most current version of MILWAUKEE SPEC CHECKLIST to include the HARDWARE SPEC GUIDE. |
| PD | Coordinate with BioMed and users+ for review on Wanderguard, Ceiling Lifts, Nurse Call, Temp Track and all new medical equipment. Review potential locations on 35% SDs. Arch plans and reflected ceiling plan need to be coordinated. Nurse Call Requirements: confirm where pull stations are required, dome/zone lighting, pillow speakers, TV connections, and master (annunciator) stations. |
| PD | If Bid Deducts are part of the project - these need to be identified in AE SOW and therefore part of the AE's proposal. Bid Deducts require FM Leadership and TMT approval. |
| PD | CADD/Revid/BIM Standards - coordinate with VA CADD/GIS Department on providing design standards and reference documents, as-builts, utility maps. Forward CAD/Revit files to for review by FM Eng Tech for compatibility, layers, plot styles, VA standards. |
| PD | If a Charrette Meeting/Report are required - update SOW accordingly. AE shall be responsible for all survey & investigations, development of the questionaire, option renderings and report. Charrette Planning Meeting can be same day as Design NTP meeting. |
| PD | Commissioning. Will the project require commissioning? As of June 2022 GLAC has BPA for CX services via task orders. If so - do you want CXA to provide CX services during design? If so, CX procurement package starts same time as AE procurement package. If not, CX procurement package will begin with construction procurement package. If no CX needed, Div 1 CX spec probably not needed, but individual MEP CX specs may be required for HVAC, Plumbing, Elec. All mechanical (AHU/controls) systemes will require CX. Others may/may not depending on Shops and FM Leadership input. Discuss with FM Leadership and AE whether Div 1 Commissioning Spec is required and if VA will procure CXA services via a third party contract. |
| PD | SOW validation meeting with AE once they have RFP from GLAC. Keep record/notes of SOW validation meeting and whether AE declines. |
| PD | VFCI Items/Equip: Discuss in detail VA provided equipment/furniture that is Contractor installed (VFCI). AE shall have a detailed schedule/table in the drawings making these items clear. COR to review VA provided toilet room/exam room equipment with EMS and ensure updated cut sheets are provided to AE. This includes Paper Towel Dispensers (eMotion or tri-fold), Sharps Containers, Soap Dispensers (liquid or foam), Hand Sanitizer Dispensers, Glove Box Holders, Toilet Paper Dispensers and Sanitary Napkin Boxes. Other items to consider: TV mounts, computer mounts, mirrors, grab bars, exam room wall panels (otoscopes). See FM PROJECT MANAGEMENT > 05-Design > 02-Equipment (VFCI). |
| PD | Geotechnical Engineering. Is sampling/testing Included? Soil borings, soil characteristics, geoprobes… If a full geotechnical report is required it must be a part of the specifications. This affects structural design and is required anytime we are moving earth. Ensure AE SOW mandates they produce all structural design. |
If we are turning over any soil on the project please ensure AE SOW considers pot-holing or engineering investigations to identify buried infrastructure/utilities. Work with Shops/CADD/GIS Dept on utility locates. Consider FLOWABLE FILL to replace native (always assume native soils are contaminated) soil - especially under any flatwork, roadway, asphalt, curb/gutter or other area where structural fill is beneficial.
If soil borings are required, the Geotechnical Report shall be included as an attachment to the Div 31 specifications. Ensure soil borings are at least 10 feet below lowest elevation of footing, caisson, pier, piling, slab, or other adjacent foundation structure. Include a dewatering clause for all contracts. Clause to be worded accordingly for the type of earthwork required that the contractor is solely responsible for this work.
| PD | Will there be new Kiosks (Veteran Check in Stations)? Coordinate with Darcel Ehley and Master Planner. |
| PD | COR will complete the NEPA document in the Planning Folder with GEMS assistance. |
| PD | COR shall begin the Risk Baseline - share with AE - update throughout design |
| PD | Review format of Submittal Register/Log in Excel with Opcenter CORs to ensure it is compatible with Opcenter. AE SOW should state that creation of the Submittal Register/Log is the responsibility of the AE. Format provided by VA. |
| PD | Coord with Users+ for survey of existing space(s) for equipment to move to new space and prepare list of new equip needed. BioMed shall input if Wanderguard, Nurse Call, Ceiling lifts, TempTrack, and new med equip are required. EMS and Logistics needs to be involved as well to consider what can be salvaged and/or re-used in new design (if practical). OI&T get involved re: new computers and phones. Note - Kay/EMS will procure CPU hangers (under desks) upon coordination with OI&T. Talk to Users about printers and RICOHs and fax machines. Send users+ Pre-Charette checklist prompting initial thoughts on type and quantities of equipment for relevant space. |
| PD | Review work in data closets (Telecommunications Rooms) with OI&T/Telecom. Will new switches be required? |
| 01 | TIL: Ensure AE downloads most current version of VA Specifications and Design Manuals from VA TIL. |
| 01 | COR/Project Manager is responsible for all applicable Division 1 specifications with exception of 01 42 19 and 01 45 29 (review AE SOW). COR/Project Manager is also responsible for Specification 02 82 11. Coordinate abatement activities spec with AE's Hazmat drawings and GEMS review. Review that part of the AE SOW Template and update accordingly if modified. |
| 01 | Infection Control. VA will provide overall classification of construction with regards to IC. Based on "type" of construction (A/B/C/D) and "patient risk group" (low/medium/high/highest), VA will determine CLASS of construction based on IC matrix (Class I/II/III/IV). This may be for overall construction or by phase. Phasing is very important if there are different classes of construction. VA and AE to coordinate placement/design of IC barriers on drawings when needed. |
| 01 | IC/CN Barriers - Fire rated plastic will only be allowed to be used for a five (5) continuous calendar days (including days not worked such as weekends). |
| 01 | IC/CN Barriers - consider the use of adjustable/mobile hard construction barriers (like Edge Guard) to create IC/CN barriers around jobsites. They are great for ante-rooms. They do not have a 1-hour fire-rating however. |
| 01 | Specifications - COR is responsible for the following specifications: 01 00 00, 01 01 10 SN, 01 32 16.15, 01 33 23, 01 35 26, 01 57 19 and 01 74 19. Work with GEMS for 01 57 19 and 01 74 19 and ensure sample CWMP is added to 01 74 19. Ensure dates in these Division 1 specs match the pre-construction timeline in the Construciton Procurement Package SOW document for GLAC. |
| 01 | Provide Opcenter access to AE in project folder for DESIGN document file transfer - from reference documents to aid in proposal to receipt of AE deliverables and sharing of VA comments. |
| 02 | Hazardous Survey in AE SOW? Hazardous materials bulk sampling is to include a materials sample from every wall in the room and the ceiling, at a minimum. Contractor sampling asbestos to identify homogeneous area and identify areas to be abated based on OHSA and WDNR interpretations. ACM/Lead - Testing and Sampling by AE hired consultant? Coordinate with GEMS, VA Safety, and A-Team. Consider scope of hazardous sampling. This, too, needs to be detailed in depth in AE SOW. |
| ACM / Hazardous abatement - if there's hazmat to abate, the Division 2 specs are required and a deep review of this spec is required by GEMS, A-team, COR and AE to ensure specification requirements match the drawings. Consider lead stabilization and not lead abatement. | |
| 02 | Lead stabilization. We tend to not include lead paint abatement specifications. Demo activities will resolve lead paint issues if found during AE specialty consultant hazmat survey. Lead STABILIZATION specification to be inlcuded if existing lead-painted walls/ceilings are to be impacted. Coordinate with GEMS, VA Safety and A-Team. |
| 02 | ACM abatement - regarding dielectric unions on perimeter heating/cooling HVAC piping system: For the replacement of dielectric unions, assume two (2) per perimeter riser. Replace dielectric unions with full-port ball valves. COR is responsible for spec 02 82 11. |
| 06 | Wall blocking (06 10 00; 09 22 16); add note as applicable in drawings as well for as much wall blocking as necessary. Provide details and notes for contractor to install wall blocking for all wall hung equipment, devices, items, etc., to include blocking for wall-mounted door stops (Div 08 71 00). | 08 | AE shall ensure that CONSTRUCTION DOORS are included in the contractor's door/hardware requirements and shall be on the hardware/door schedule. Infection Control/Construction doors shall be fire-rated (1hr), hollow metal doors, with closing devices as required by NFPA 241 and the Safety and Special Notes Specifications. |
| 08 | Doors/Frames/Hardware - ensure AE has updated spec guides. Have discussion with FM Leadership/Arch Shop/EMS/Users re: hospital stop vs regular stop frames. When door frames with hospital (terminated) stops are installed, a 6" cove base will be used. |
| 08 | Stairwell doors - coordinate changes to stairwell doors (hardware, access control) with VA Locksmith, Users and Fire Marshall/Safety. |
| 08 | Hardware/Auto Openers/Card Readers: Coordination between Div 08, 26 and 28 is paramount in the drawings. Who supplies power supplies for electrified hardware (strikes, openers, panic bars, card readers, etc.)? Arch/Elec Power/Elec Tech sheets and hardware schedules need to be coordianted. |
| 08 | Hardware: AE needs to review the current VA Patient Privacy and Dignity Design Guide. |
| 08 | Hardware: AE shall work with the COR and VA Locksmith to prepare a core schedule. This must be created prior to 100% deliverable, and will be completed during the construction hardware submittal phase. This core schedule shall include doors, access doors, millwork and cabinetry: all doors receiving cores. |
| 08 | Hardware: Specify Mortise Locksets and Latches (Sargent Series 8200) for all hardware with the exception of toilet rooms (Cylindrical OK). |
| 08 | You will need a separate Hardware meeting between 35% and 65% with Users, AE, AE hardware consultant and VA Locksmith. Result of this meeting is the hardware pinning/keying schedule. Ensure hardware schedule is clear: GC provides all hardware listed in each group, however SFIC cores are shipped directly to VA Locksmith. COR is responsible for Medeco letter during construction. Cores shall be pinned/keyed to the keyway/series as prescribed by a provided core spreadsheet during construciton submittals. AE shall provide a narrative in hardware schedule for each door regarding how it opens/how you ingresss/egress through the door. Sample spreadsheet is next tab. |
| 08 | Exterior louvers (08 90 00) to match existing architecture of building (including color). All louvers to have insect screen. |
| 09 | Review with COR and Interiors Designer for need for a color board for each exterior and interior design work. |
| 09 | There shall be no carpeting incorporated into any renovations. AE shall coordinate with VA Interiors for flooring materials. |
| 09 | All interior finishes/colors/styles and materials are reviewed by Interior Designer and EMS. This includes elevator cabs, cubicle curtains, furniture, flooring, bathroom tile, painting, wall/door protection, artwork, baseboard and sconce lighting/accessories. |
| 09 | Finishes Schedule review - Interior Designer to review finishes schedule (drawings), and specification 09 06 00. Also - if AE SOW required a finishes "sample" board, that should be received at 65% and forwarded for review. This includes flooring materials, window treatments, paint.... Consider separate meeting with Arch Shop to discuss finishes as well for spec edits based on historical maintenance issues. Coordinate with Users+ to order loose furniture for the project if needed (chairs, end tables...) |
