About this file

This is a Request for Quotation (RFQ) for floor replacement services at the Albuquerque Indian Health Center, issued by the Department of Health and Human Services, Indian Health Service (IHS).

The solicitation (75H70726Q00080) is set-aside for Indian Small Business Economic Enterprise (ISBEE) concerns with a cascading set-aside structure: first tier for ISBEE small business concerns, second tier for all other small business concerns if insufficient competition is received. The NAICS code is 238330 (Building Finishing Contractors) with a $19.0 million size standard. The project involves removal and replacement of deteriorated carpet and vinyl composition tile (VCT) flooring in the Health Information Management Department covering approximately 1,330 square feet. The scope includes asbestos abatement of confirmed asbestos-containing mastic beneath the VCT, subfloor preparation with leveling and crack repair, and installation of commercial healthcare-grade sheet vinyl flooring with heat-welded seams and integral cove base. An alternate finish of polished concrete (800-1500 grit) is available for evaluation based on slab condition and cost comparison. The facility will remain occupied during performance. The contract is firm-fixed price (FFP) with a base period of six months or less (TBD/2026–TBD/2026) and includes an option to extend services for an additional six months at prevailing labor rates. The offer due date is August 7, 2026, at 5:00 PM Mountain Daylight Time, with site visits available until July 28, 2026, at 4:30 PM. Quotations must be organized into three volumes: Technical Qualifications, Past Performance (up to five recent contracts), and Price. Award will be made on a best-value basis with Technical Capability and Past Performance weighted more heavily than Price. Invoices must be submitted electronically through the Department of Treasury Invoice Processing Platform (IPP) at www.ipp.gov, and the contractor must include a W-9 form and Unique Entity Identifier (UEI) on each invoice.

However, the Government will consider the certification in connection with a determination of the Offeror's responsibility. Failure of the Offeror to furnish a certification or provide such additional information as requested by the Contracting Officer may render the Offeror nonresponsible.

(d) This provision does not require establishment of a system of records in order to render, in good faith, the certification required by paragraph (a). The knowledge and information of an Offeror is not required to exceed that which is normally possessed by a prudent person in the ordinary course of business dealings.

(e) The certification in paragraph (a) of this provision is a material representation of fact upon which reliance was placed when making award. If the Government later determines that the Offeror knowingly rendered an erroneous certification, in addition to other remedies available to the Government, the Contracting Officer may terminate the contract resulting from this solicitation for default.
