QUOTATION SUBMISSION

STATEMENT OF WORK FOR WELL PREVENTATIVE MAINTENANCE

1. GENERAL INFORMATION

a. Introduction: Rehabilitate and preventative maintenance on wells at the USGS

Upper Midwest Environmental Sciences Center (UMESC); 2630 Fanta Reed Road, La Crosse, WI 54603.

b. Background: The UMESC facility complex in La Crosse, WI, maintains field study projects and programs. The three wells at this facility require annual preventative maintenance and inspections in order to maintain non-potable water supply to the Center. Inspection of Well #3 screens is required to determine if cleaning is necessary, since the well was last cleaned in 2017.

c. Scope: The intent of this Contract is to perform preventative maintenance services on the three wells serving UMESC primary research water supply.

Additional inspection and repair services should the Government choose to exercise the option given the additional need for allowing the well to produce increased water flows at higher pumping levels and approach its design output.

i. The Contractor shall provide all necessary equipment, labor, and tools required to perform the work as identified in this SOW.

2. WORK REQUIREMENTS

a. Request for Quotation (RFQ) package

i. Contractor shall provide a Base Quote to perform the following services identified in this PWS:

1. Preventative Maintenance Services – Wells 1, 2, and 3

2. Camera Inspection Services – Well #3

ii. Contractor shall provide pricing for options to perform the following should it be determined that additional repair services are necessary:

1. Well Cleaning Services

2. Motor/Pump Rehabilitation Services

iii. Contractor shall provide, as part of the quote, unit costs for labor, travel and representative material costs for all work associated with this SOW.

iv. The Contractor shall include the following as part of Contractor’s Bid:

1. SDS sheets with descriptions of any/all chemicals to be used for this SOW.

2. Equipment technical specifications for each piece of furnished equipment.

3. Itemized equipment list to include price, model, quantity and description of all furnished equipment.

4. Itemized breakdown to include labor, travel, per diem, warranties, freight charges, supervision and documentation associated with this work.

v. Contractor shall provide pricing for option years to provide Preventative Maintenance Services and Camera Inspection Services for each year of successive of 4 years of service should the Government choose to exercise additional years of service with the Contractor.

b. Site Visit: Offerors or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award.

c. Service Requirements: Work shall involve at a minimum the following:

i. Preventative Maintenance Services

1. Inspection of components: Valves, Pump Base, Airlines, Etc.

2. Report on Existing Pump Performance, Comparison with Design

3. Pump Vibration Inspection/ Assessment

4. Recording of Motor Electrical Data (Running Amps, Voltage)

5. Changing of Motor Oil and Gear Drive Lubricants

6. Repacking of Stuffing Box

7. Provide pump installation sheets and hydraulic data sheets

UMESC facility Technical Liaison (TL).

ii. Camera Inspection Services

1. Well shall be televised to inspect the current condition of the well casing and screen. Provide recommendations on the rehabilitation procedures. Provide video inspection footage to UMESC facility Technical Liaison (TL).

iii. Optional Rehabilitation Services

1. Well Cleaning Services

a. Contractor shall coordinate with and receive approvals from the Wisconsin Department of Natural Resources as required prior to any Work being performed

b. Complete approved well rehabilitation of well with chemical/Aqua-Freed process:

i. Measure existing well water pH

ii. Wire brush well casing and screen to remove encrustation of iron and manganese deposits

iii. lnstall air lift piping and complete removal of debris accumulated from wire brushing

iv. Remove air lift píping and install Aqua-Freed equipment to complete rehabilitation

v. lnject a 3,000-gallon acid mixture in the well

vi. lnject 5 tons of liquid CO2 into the well screen.

vii. Flush well until discharge pH returns to 8.2

viii. Televise well to confirm improvement of well screen.

ix. Provide completed before/after camera video inspection footage, pump installation sheets and hydraulic data sheets to UMESC facility Technical Liaison (TL).

x. Water pH levels for the well before and after treatment will be documented as part of the Contractor report.

2. Motor/Pump Rehabilitation Services:

a. Remove pump motor and well pump unit complete and load out for cleaning and inspection/repairs.

d. Technical Requirements

i. Work shall be performed during regularly scheduled facility hours (7:30am-4pm; Mon. – Fri.). However, if facility personnel will be negatively impacted by the work, after hours and weekend work may be authorized. Any work performed by the Contractor outside of normal facility hours must be coordinated with the USGS TL at least 48 hours in advance.

ii. Contractor shall be capable of providing on-staff technicians factory-trained in handling all Contractor furnished material and equipment.

iii. Contractor vehicles used for this work shall be staged on an asphalt surface, no closer than 20 feet from any drainage ditches.

iv. Coordinate work with the USGS TL to ensure any facility equipment or research is not interrupted or disturbed. Contractor shall be responsible for any work performed as part of this contract that causes damage to existing equipment, harms aquaculture livestock or negatively impacts ongoing research.

v. Contractor shall provide equipment as necessary to minimize debris in and around work areas. Contractor shall clean surfaces or equipment impacted by construction work and remove any debris created by the work associated with this project.

vi. Contractors will notify the USGS TL prior to entry into other areas of the facility not directly related to this project scope. Entry into offices, mechanical/electrical spaces and or laboratories without permission from USGS TL is prohibited.

vii. Any issues affecting project completion or delivery dates shall be communicated to the USGS TL as soon as practical but no later than 24 hours.

viii. Provide report to UMESC after completion of Work.

ix. The installing contractor shall submit the following information and drawings for approval prior to starting installation work on this project:

1. SDS Sheets: Submit OSHA-approved Safety Data Sheets outlining dangers, exposure treatments and safe handling instructions for all material to be used as part of this Project.

2. Outline of Contractor’s process for the preventative maintenance, inspection services for the 3 wells, and optional services should they be required. Include all processes and specific steps in addition to the work outlined in this SOW.

e. Project Completion Documentation:

i. The Contractor shall furnish, at a minimum, two (2) complete sets of manuals with a table of contents. These manuals at a minimum will include the following information:

d. Special Considerations

i. The installing contractor shall show proof of emergency service available 24-hours-a-day, 7-days-a-week.

ii. The contractor shall comply with agency personal identification procedures identified in the contract that implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Personnel and Management (OMB) guidance M-05-24 and Federal Information Processing Standard Publication (FIPS PUB) Number 201.

iii. The Contractor shall account for all forms of Government-provided identification issued to the to the Contractor employees in connection with performance under this contract. The contractor shall return such identification to the issuing agency at the earliest of any of the following, unless otherwise determined by the Government:

1. When no longer needed for Contract performance

2. Upon completion of the Contractor employees employment

3. Upon contract completion or termination.

iv. If after hours work is required to be performed, Contractor will have at least one (1) vetted employee at the UMESC facility either performing work or in a direct supervisory role as part of this contract while on-site installation work as part of this project is being performed by the contractor.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.

Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1), in a subcontract for commercial products or commercial services. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause-
