About this file

This is a Snow Plow Evaluation Checklist developed by the Department of Defense, Veterans and Emergency Management—Military Bureau for assessing contractor performance on snow plowing and sanding services at designated facilities in Maine, including the Skowhegan Armory. The checklist establishes a standardized evaluation framework to be conducted on a monthly basis at minimum, with additional evaluations scheduled by the Program Administrator with three business days' notice. Evaluation tours assess contractor compliance across general tasks including snow and ice removal from parking areas, roads, sidewalks, and designated entrances when accumulations reach two inches or more; treatment of roadways and parking lots during plowing operations; removal of unsafe ice conditions through sanding and salting; and completion of work by 6:00 a.m. The checklist includes facility-specific requirements such as 24/7 operations support at Bangor Flight Facility Building #260, potential 24/7 operations at Bangor RTI #700, and shoveling services for walkways and handicap ramps. Contractors must also manage snow pile locations, prevent drift creation at critical areas such as fire lanes and hydrants, complete daytime snow pile movements where required, and maintain contact information for on-call service requests.

Contractor performance is rated using an Excellent, Good, Needs Improvement, or Not Applicable scale. Poor performance is documented when a contractor receives four or more negative responses on yes/no questions or Needs Improvement ratings during a single evaluation tour. Contractors have two weeks from the evaluation date to correct identified deficiencies; failure to resolve issues within this timeframe may result in contract termination with thirty days' notice. Three poor performance evaluations across service tours will trigger automatic contract termination. Contractors have fifteen calendar days to appeal any termination decision to the Director of the Division of Procurement Services. The evaluation process requires documentation of pre-existing damage within thirty days of contract start, photographic evidence of work completion, completion of repairs to damaged areas, and timely response to all communications within one business day. Evaluation tour attendees must include facility contacts, DFE representatives, and vendor/provider contacts, with completed checklists returned to the designated DFE contact.
