About this file

This is a Snow Plow Checklist and Service Tour Evaluation form for the Department of Defense, Veterans and Emergency Management - Military Bureau related to snow plowing and sanding services at the Bog Brook Training Site in Maine. The checklist establishes performance standards and evaluation procedures for contractors providing snow removal and sanding services at designated facilities. Contractors are required to clear and sand designated areas, including parking lots, entrances, roads, and sidewalks, when precipitation accumulations reach two inches or more, with all areas reasonably cleared no later than 6:00 AM. Special requirements vary by facility, including 24/7 operations at Bangor Flight Facility Building #260 and potentially at Bangor RTI #700, as well as shoveling services for walkways, handicap ramps, and stairs at RTI and Camp Keyes. The contract requires monthly performance evaluation tours at minimum, with additional evaluations scheduled with three business days' notice.

The evaluation criteria are structured to document contractor performance through yes/no questions and ratings of Excellent, Good (Meets Expectations), and Needs Improvement. Poor performance is defined as four or more negative responses or unfavorable ratings, and contractors receive two weeks to correct identified issues. Contractors receiving three poor performance evaluations face potential contract termination with thirty days' notice. Contractors have fifteen calendar days to appeal termination decisions to the Director of the Division of Procurement Services. Additional requirements include providing photographic documentation and detailed damage assessments within thirty days of contract start, maintaining appropriate contact information for weekend and holiday operations, responding to communications within one business day, and completing snow pile movement and removal during daytime hours. All evaluation forms must be completed and returned to the designated DFE contact.
