About this file

This is a Snow Plow and Sanding Service Evaluation Checklist prepared by the Department of Defense, Veterans and Emergency Management, Military Bureau for the Gardiner Training Site in Maine. The document establishes performance evaluation standards and procedures for contractor snow removal and sanding services. The contract requires contractors to clear and sand designated areas including parking lots, entrances, roads, and sidewalks within specified timeframes, with all parking areas and access roads to be cleared no later than 6:00 AM following precipitation events of two inches or more. Performance evaluations are to be conducted monthly at minimum, with additional evaluations scheduled with three business days' written notice. Special requirements apply to specific facilities, including 24/7 operations at Bangor Flight Facility Building #260 and Bangor RTI #700, where contractors must respond to service requests at any time during day or night based on notification from facility points of contact or security personnel.

The evaluation framework establishes clear performance standards with contractors receiving ratings of Excellent, Good (Meets Expectations), Needs Improvement, or Not Applicable across multiple service categories. Contractors receiving four or more negative responses or poor performance ratings will be given two weeks to correct identified issues, with contract termination possible after thirty days' notice if issues remain unresolved. Three documented poor performance evaluations trigger automatic contract termination rights. Contractors may appeal termination decisions within fifteen calendar days to the Director of the Division of Procurement Services. Additional contractor obligations include providing photos and damage documentation within thirty days of contract start, maintaining contact information for on-call service requests, responding to all communications within one business day, and completing snow pile removal and lawn repairs as necessary. Evaluation tours require attendance and sign-off from facility contacts, DFE contacts, and vendor representatives, with completed evaluation sheets submitted to Ken Merrifield at kenneth.r.merrifield.nfg@army.mil.
