About this file

This is a Snow Plow Checklist and service evaluation document used by the Department of Defense, Veterans and Emergency Management - Military Bureau to assess contractor performance on snow plowing and sanding services at Augusta Armory and other designated Maine facilities. The checklist establishes performance standards requiring contractors to clear and sand designated areas including parking lots, entrances, roads, and sidewalks when precipitation accumulates two inches or more, with all parking areas, access roads, and sidewalks to be cleared no later than 6:00 am. Special operational requirements apply to specific facilities such as Bangor Flight Facility Building #260 and Bangor RTI #700, which may require 24/7 snow removal and sanding operations upon notification by facility point of contact or security personnel. The contractor must respond to all communications within one business day, provide photographic documentation of pre-existing damage within 30 days of contract start, and maintain specified snow piling areas while preventing drifts at fire lanes, dumpsters, and fire hydrants.

Performance evaluations are conducted monthly at minimum, with the contractor receiving poor performance ratings if four or more negative responses are documented or if ratings indicate needs improvement or unacceptable service levels. Contractors receiving two weeks to remedy identified deficiencies may face contract termination with 30-day notice if issues persist, and three poor performance evaluations result in automatic termination eligibility. Contractors have 15 calendar days to appeal termination decisions to the Director of the Division of Procurement Services. The checklist requires signed attendance from facility contacts, DFE representatives, and vendor contacts, with completed evaluation forms submitted to Ken Merrifield at the provided email address.
