About this file

This document is a Performance-Based Quality Assurance Surveillance Plan (QASP) for a Sharps Waste Management Services contract. The matrix outlines six key performance areas: employee competency, safety, facility service performance, invoice accuracy, report timeliness and accuracy, and issue resolution. For each area, the document specifies standards, quality assurance methods, and financial incentives/penalties.

The QASP includes specific performance requirements with corresponding deductions for non-compliance, such as a 5% monthly invoice reduction for using unapproved employees, 1% deduction for safety incidents exceeding two per year, 2% reduction for service schedule violations, and 3% deduction for unresolved problems after seven working days. Quality assurance will be conducted through periodic inspections, 100% inspections, customer input, and reports of injuries/accidents. The matrix provides a structured approach to measuring and maintaining contractor performance, with clear consequences for failing to meet established standards.
