The 375th Contracting Squadron (PKB2) at Scott AFB, IL is contemplating an award for the purchase of CCTV Camera Preventive Maintenance Service. Quotes are for four (4) quarterly PMI and annual total for base. There is also four option periods to include a 6-month extension of services. The reimbursable CLIN will have an amount added annually that will be used to pay for any broken equipment and does not require you to bid. Please quote per year and provide a grand total for the entirety of the contract.
This is a commercial purchase using FAR Part 12 and Part 13 procedures. The following information, as required per FAR 12.603(c) (2) applies:
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued.
(ii) The solicitation reference number is FA4407-22-Q-MS01; it is being issued as a Request for Quotation (RFQ).
(iii) The solicitation document, incorporated provisions, and clauses are those in effect through the Federal Acquisition Circular 2022-04 effective 30 January 2022, DFARS 205.203 (S-70) and DFARS Public Notice 2016-D006.
(iv) This acquisition is being solicited as a 100% Small Business Set Aside on SAM.gov webpage utilizing NAICS Code 811211, Consumer Electronics Repair and Maintenance, with a PSC Code of J059, Maintenance/Repair/Rebuild of Equipment—Electrical and Electronic Equipment Components.

This is a non-personal services contract to provide quarterly Preventive Maintenance Inspection (PMI) of Scott AFB cameras system to include all other associated equipment such as digital recording devices, camera housing units and cabling. The contractor will also be responsible for service responses for unscheduled camera and associated equipment outages as outlined in the PWS. The Government shall not exercise any supervision or control over the contract service providers performing the services herein.
Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. (See PWS)
PROVIDE PRICING FOR EACH CLIN WITH YOUR RESPONSE.
(vi) Description of requirements: CCTV Camera Preventive Maintenance Service to be performed IAW attached PWS.
(vii) The Government desires FOB Destination: Performance will be on Scott AFB, IL.
(viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Nov 2021) is hereby incorporated by reference, with the same force and effect as if it were given in full text. In addition to the following addendum, all terms and conditions of FAR 52.212-1 remain in effect.
The following is tailored to this acquisition and is hereby added via addendum:
1. To assure timely and equitable evaluation of the offer, the offerors must follow the instructions contained herein. The offer must be complete, self-sufficient, and respond directly to the requirements of this solicitation.
2. Specific Instructions: The response shall consist of two (2) separate parts:
Part 1 - Cover Letter – The Cover Letter shall include the Offerors points of contact with e-mail address and phone number who are authorized to contractually obligate the company, CAGE Code, DUNS Number, small business status/categories, a statement the company understands the requirements specified and will meet the performance standards and requirements therein, and a statement the company does or does not take exception to any of the requirements of this solicitation.
Part 2 – Price – (Evaluation Factor 1) - Complete paragraph (v) of this solicitation. Price will be evaluated for (i) Total Evaluated Price (TEP) (ii) completeness (iii) reasonableness and (iv) unbalanced pricing. Offerors whose price is determined to be unreasonable or incomplete will not be considered for award. The burden of proof for credibility of proposed prices rests with the offeror.

Passed by Congress in 2005, the REAL ID Act enacted the 9/11 Commission's recommendation that the Federal Government "set standards for the issuance of sources of identification, such as driver's licenses." Due to circumstances resulting from the COVID-19 pandemic and the national emergency declaration, the Department of Homeland Security, is extending the REAL ID enforcement deadline until October 1, 2021.

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

The following is tailored to this acquisition and is hereby added via addendum:
BASIS FOR CONTRACT AWARD: This is a competitive simplified acquisition selection conducted in accordance with Federal Acquisition Regulation (FAR) Part 13. The Government will select the best offer which conforms to the solicitation requirements (to include all stated terms, conditions, representations, certifications, and all other information required by the submission instructions), and is judged, based on the evaluation factors to represent the best value to the Government. The Government seeks to award to the offeror who is best suited to fulfill the requirement. This may result in an award to a better value, higher priced offeror, where the decision is consistent with the evaluation factors, and the Government reasonably determines that technical approach of the higher priced offeror outweighs the price difference. The Government will review all documents for compliance.
Once the Government determines the Offeror that is best-suited to meet stated requirements, the Government reserves the right to communicate with only that Offeror to address any issues, if necessary, and finalize an order with that Offeror. If the parties cannot successfully resolve outstanding issues, as determined pertinent at the sole discretion of the Government, the Government may communicate with the next best-suited Offeror, based on the original analysis and address any necessary issues with that Offeror. If the Government begins communications with the next best-suited Offeror, no further communications with the previous Offeror will be http://www.acquistion.gov/ entertained until after order award. This process will continue until an agreement is successfully reached and an order is awarded.
Factor I: Price
Under this factor, the Government will evaluate the Offerors Price based on the following:
1. Offers should support the proposed technical approach and will be evaluated for (i) Total Evaluated Price (TEP), (ii) completeness, (iii) reasonableness and (iv) unbalanced pricing.
Offerors whose price is determined to be incomplete or unreasonable will not be considered for award.
2. The TEP will be calculated as the sum of the Offerors proposed price for contract line item numbers 0001-0003. The TEP will be evaluated for completeness, reasonableness, and unbalanced pricing to arrive at the TEP. Offerors whose price is determined to be incomplete or unreasonable will not be considered for award.
2.1. Completeness. The offer will be reviewed to determine if all price elements have been addressed. The review will determine the adequacy of the offerors offer in addressing and fulfilling the requirements.
2.2. Reasonableness. For a price to be reasonable, it must represent a price to the Government that a prudent person would pay in the conduct of competitive business. The Government will determine prices fair and reasonable through the use of one or more of the price analysis techniques at FAR 13.106-3(a).
2.3. Unbalanced pricing. Offerors are cautioned against submitting an offer that contains unbalanced pricing. Unbalanced pricing may increase performance risk and could result in payment of unreasonably high prices. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one (1) or more contract line items is significantly over or understated. The Government shall analyze offers to determine whether they are unbalanced with respect to separately priced line items. An offer that is determined to be unbalanced may be rejected if the Contracting Officer determines that the lack of balance poses an unacceptable risk to the Government.
(x) Offerors must include with their offer a completed copy of the provision at 52.212-3, Offeror Representations and Certifications—Commercial Items (Feb 2021), unless this information has been submitted electronically as part of its annual representations and certificates at https://www.sam.gov.
As prescribed in 52.212-3, an offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically in the System for Award Management (SAM) accessed through https://beta.sam.gov. If an offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (v) of this provision.

The Government may require continued performance of any services within the limits and at the rates specified in the contract . These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months.

As prescribed in 52.107(f), insert the following clause in solicitations and contracts that include any FAR or supplemental clause with an authorized deviation. Whenever any FAR or supplemental clause is used with an authorized deviation, the contracting officer shall identify it by the same number, title, and date assigned to the clause when it is used without deviation, include regulation name for any supplemental clause, except that the contracting officer shall insert “(DEVIATION)” after the date of the clause.

52.217-9 Option to Extend the Term of the Contract.
As prescribed in 17.208(g), insert a clause substantially the same as the following:
OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within _15__ [insert the period of time within which the Contracting Officer may exercise the option ]; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least __30_ days[60days unless a different number of days is inserted] before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed __66__ (months) (years).
5352.201-9101 AMC Ombudsman 10/1/2019
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(a) Contractors shall not:
(1) Provide any service or product with any specification, standard, drawing, or other document that requires the use of a Class I ODS in the test, operation, or maintenance of any system, subsystem, item, component, or process; or
(2) Provide any specification, standard, drawing, or other document that establishes a test, operation, or maintenance requirement that can only be met by use of a Class I ODS as part of this contract/order.
(b) For the purposes of Air Force policy, the following products that are pure (i.e., they meet the relevant product specification identified in AFI 32-7086) are Class I ODSs:
(1) Halons: 1011, 1202, 1211, 1301, and 2402;
(2) Chlorofluorocarbons (CFCs): CFC-11, CFC-12, CFC-13, CFC-111, CFC-112, CFC- 113, CFC-114, CFC-115, CFC-211, CFC-212, CFC-213, CFC-214, CFC-215, CFC-216, and CFC-217, and the blends R-500, R-501, R-502, and R-503; and
(3) Carbon Tetrachloride, Methyl Chloroform, and Methyl Bromide.

HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (OCT 2019)
(a) In performing work under this contract on a Government installation, the contractor shall:
(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and
(2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.
(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.
(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

5352.242-9000 Contractor Access to Air Force Installations
As prescribed in AFFARS 5342.490-1, insert a clause substantially the same as the following clause in solicitations and contracts:
CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (OCT 2019)
(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate, and [insert any additional requirements to comply with local security procedures] to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [insert any additional requirements to comply with AFI 31-101, Integrated Defense, and DODMAN5200.02_AFMAN 16-1405, Air Force Personnel Security Program] citing the appropriate paragraphs as applicable.
(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.
(f) Failure to comply with these requirements may result in withholding of final payment.
