About this file

This is a Services Summary Table (SST) that establishes performance standards and quality metrics for mortuary services supporting the VA Salt Lake City Health Care System's four-year Blanket Purchase Agreement (BPA) for deceased unclaimed or indigent Veterans. The contractor must provide comprehensive mortuary services including removal, preparation, storage, cremation, embalming, casketing, transportation, and interment confirmation for deceased Veterans from the George E. Wahlen VA Medical Center and other VA-authorized care facilities in Utah, with delivery to designated state and federal Veterans cemeteries including the Utah Veterans Cemetery & Memorial Park at Camp Williams and Southern Utah National Cemetery.

The SST establishes specific performance requirements with 100% compliance standards for critical operations: remains must be picked up within one business day of VA notification; held for a minimum 14 calendar days (or longer per state/local law); and burial or inurnment completed within five business days of receiving the Proceed Notice. Additional requirements include secure and sanitary refrigerated storage; proper cremation with sealed, durable, and labeled urns; body preparation per industry standards and applicable law; caskets meeting minimum specifications (20-gauge metal or thicker); and proper flag placement for burials. The contractor must maintain current professional licenses and permits (including transit, cremation, and burial permits), ensure facility compliance with NFPA Fire & Life Safety Code and VA accessibility standards, establish and submit a Quality Control Program with semi-annual reports, implement corrective action plans within three calendar days (one business day for health/safety issues), protect personally identifiable and protected health information per VA policy, and complete annual privacy and security training. The Contracting Officer's Representative (COR) maintains inspection rights over remains, caskets, urns, and facilities (including subcontractor locations) and may conduct random and unscheduled inspections to verify compliance.
