About this file
This is a Quality Assurance Surveillance Plan (QASP) for mortuary services provided to the VA Salt Lake City Health Care System. The QASP establishes the Government's methodology for monitoring contractor performance against the Performance Work Statement (PWS v3) requirements for cremation and burial services for deceased unclaimed or indigent Veterans in Utah. The plan defines roles and responsibilities for the Contracting Officer, Contracting Officer Representative (COR), Technical Subject Matter Experts, and Station Quality Assurance personnel who will oversee contractor compliance.
The QASP outlines six primary surveillance methods: random inspections of mortuary operations, scheduled inspections aligned with service milestones, customer feedback collection, document reviews, onsite facility inspections, and case timeline tracking. Key performance requirements include a 1-business-day response time for remains removal, minimum 14-calendar-day holding periods, 5-business-day burial/inurnment completion following the Proceed Notice, proper refrigeration and storage, appropriate body preparation and embalming when applicable, and secure transportation to Utah Veterans Cemetery & Memorial Park at Camp Williams or other designated Utah cemeteries. All performance requirements mandate 100% compliance with zero violations for confidentiality and privacy handling. The COR conducts pickup timeliness verification at every occurrence, burial/inurnment completion timeline verification at every occurrence, facility inspections as needed, and annual license and permit verification. Documentation is maintained through inspection logs, case timeline logs, deficiency reports, and monthly performance summary reports. When deficiencies occur, the contractor must submit a Corrective Action Plan within a specified timeframe, with COR follow-up verification. The QASP may be updated when the PWS changes, performance issues arise, or new high-risk areas are identified.
