About this file
This is a Sensitive Security Information Acknowledgement Form issued by the City of Phoenix Aviation Department as Attachment E to a solicitation for automatic transfer switch (ATS) testing, maintenance, and repair services. The City of Phoenix is soliciting sealed offers for a three-year contract with two optional one-year extensions, commencing on or about June 1, 2026. Services encompass testing, maintenance, and repair of over 130 ATS units deployed across multiple municipal departments including Fire, Information Technology, Libraries, Parks and Recreation, Police, Public Works, and Street and Transportation. A non-mandatory pre-offer conference is scheduled for January 13, 2026, at 10:00 am, with written inquiries due by January 19, 2026, at 3:00 pm, and sealed offers due on March 12, 2026, at 3:00 pm. The contractor must possess a minimum of three years of operational experience in ATS installation, inspection, testing, troubleshooting, and repair, along with a current C-11 Electrical license from the Arizona Registrar of Contractors. The contract requires 24-hour emergency response capability with 30-minute initial response and one-hour onsite response for both emergency and non-emergency situations.
The solicitation mandates robust Disadvantaged Business Enterprise (DBE) and Small Business Participation Plan requirements, obligating contractors to demonstrate good faith efforts to engage DBE-certified and small business suppliers through tracking and monthly reporting in the City's Certification and Compliance System. Contractors must identify potential categories of goods and services available from small and DBE-certified suppliers, list current DBE firm and small business partners, and provide annual expenditure summaries. The form presented here requires authorized company representatives to acknowledge receipt and proper handling of documents containing Sensitive Security Information controlled under 49 C.F.R. Parts 15 and 1520, and to certify destruction of such documents upon contract completion or no later than the specified contract end date, in compliance with federal aviation security regulations.
