About this file
This is a Sensitive Security Information Acknowledgement Form issued by the City of Phoenix Aviation Department for use in conjunction with a state and local contract opportunity for hauling of water sludge and screening hauling services in Arizona. The form serves as a template document designed to protect sensitive security information controlled under 49 C.F.R. Parts 15 and 1520, restricting disclosure to authorized personnel with a demonstrated need to know. The contract term is established for five years beginning on the commencement date approved by Phoenix City Council, with no options to extend. The agreement will terminate upon the earliest occurrence of reaching the end of the term or pursuant to termination provisions contained within the contract.
The acknowledgement form requires authorized company representatives to sign and acknowledge their understanding of handling restrictions, including the obligation to destroy specified documents in compliance with applicable federal regulations upon completion of contract review and use or by the current contract end date, whichever is earlier. The solicitation is available through the City's Procurement Portal, with technical support provided through the portal interface or via email at procurement-support@opengov.com. The contract is valued at no specified amount and will become effective upon final signature and recording by the City Clerk's department.
