About this file
This is a Sensitive Security Information Acknowledgement Form for the City of Phoenix Aviation Department related to a hazardous detection equipment, accessories, and preventive maintenance services contract. The City of Phoenix is soliciting sealed offers for a five-year hazardous detection equipment and preventive maintenance services contract with an anticipated commencement date of June 1, 2026, or upon City Council award and City Clerk recording, whichever is later. The contract will terminate upon the earliest of: expiration of the term including any exercised extensions, or termination pursuant to the agreement provisions. Offerors must submit responses through the City's Procurement Portal, with technical support available through the portal's support feature or at procurement-support@opengov.com.
The acknowledgement form pertains to documents containing Sensitive Security Information controlled under 49 C.F.R. Parts 15 and 1520, which restricts disclosure to individuals with requisite "need to know" and subjects unauthorized release to civil penalties. Authorized company representatives must sign the acknowledgement, agreeing to destroy all controlled documents upon completion of their review and use or by the current contract end date, whichever occurs first. All recipients acknowledge their understanding that U.S. government agencies are subject to disclosure restrictions under 5 U.S.C. 552 and the referenced CFR regulations.
