About this file
The document is a Contractor Wage Compliance Statement from the Colorado Department of Transportation (CDOT) for state-funded public projects. This form is designed to ensure contractors and subcontractors comply with wage and fringe benefit requirements, with contractors required to submit the form monthly alongside their approved pay application. The form captures detailed information about payroll, including contractor and subcontractor names, payroll periods, fringe benefit details, and verification of on-the-job trainees' registration and payment according to approved training programs.
The form mandates that contractors declare under penalty of perjury that all statements are true and complete, with specific requirements for documenting wage payments, fringe benefits, and employee classifications. Contractors must provide information on fringe benefits such as cash, pension, health insurance, vacation, and other benefits, and confirm that all employees are paid full weekly wages without unauthorized rebates or deductions. The document emphasizes the importance of accurate payroll reporting and warns that willful falsification may subject the contractor or subcontractor to civil or criminal prosecution under federal statutes.
