About this file

This is a template for the Sector Representatives Table required as part of the Drug-Free Communities (DFC) Support Program grant application for FY 2026. The template instructs applicants to complete a table that documents the name, organization, and selection rationale for representatives from twelve distinct community sectors. The required sectors include youth, parent, business, media, school, youth-serving organizations, religious or fraternal organizations, law enforcement (non-retired), civic and volunteer groups, healthcare professionals or organizations, state/local/tribal government agencies with substance abuse expertise, and other organizations involved in reducing substance abuse. For youth representatives, applicants must specify the age of the youth participant. The selection rationale for each sector representative must be substantive and align with the sector representative definitions provided in the Notice of Funding Opportunity (NOFO), as these rationales will be reviewed as part of the application evaluation process.

The DFC Support Program, administered by the Centers for Disease Control and Prevention (CDC) under CFDA 93.276, awards grants to community coalitions to prevent and reduce substance abuse among youth aged 18 and younger through collaborative, evidence-based prevention strategies. Eligible applicants must demonstrate at least six months of prior coalition work on substance abuse reduction, maintain the required sector composition, secure substantial community volunteer effort, and provide a 100-150 percent cash or in-kind match. The cooperative agreement period ranges from one to five years, with typical award values between $250,000 and $500,000 annually. Recipients must submit annual Federal Financial Reports and performance reports, and participation in program evaluation is mandatory.
