About this file
This is an incident report form used by the WJHTC (William J. Hughes Technical Center) to document and investigate workplace incidents involving employees, contractors, vendors, visitors, or other personnel.
The form captures incident classification by type (near miss, injury/illness, first aid, property damage, environmental spill/release, motor vehicle, or non-work related) and whether the incident is OSHA recordable. Section 2 collects employee information including name, employee type, routing number, job title, line of business, and manager. Section 3 documents incident details including date, time, location, witness information, and a detailed narrative describing the actual incident, events leading up to it (including behavioral aspects), and possible causes. Section 4 requires identification of corrective actions taken immediately and preventative actions needed to prevent further incidents. The form includes an investigative considerations section that prompts reviewers to identify behavioral safety factors (rushing, frustration, fatigue, complacency) and critical errors (eyes not on task, mind not on task, line-of-fire, balance/traction/grip issues), as well as root cause analysis. The final section captures investigation details including the investigator's name, job title, email, phone number, and investigation date. The form provides standard definitions for incident classifications at the bottom.
