About this file
This document is an Addendum 4 to a Request for Proposal (RFP) issued by the California Department of Corrections and Rehabilitation (CDCR) and the California Correctional Health Care Services (CCHCS) for On-Site Dialysis Services solicitation number SD23-00097. The RFP requests proposals for providing dialysis services at four CDCR institutions located in Marin, San Diego, and San Joaquin counties. The RFP provides details on the current equipment, dialysis chair counts, and reverse osmosis systems at each facility, as well as the number of dialysis treatments performed. Key dates include a question response deadline of October 22, 2024, and a proposal due date of November 1, 2024.
The RFP indicates that CDCR/CCHCS will provide the awarded vendor with specific facility details, medical director and nephrologist information, and laboratory service provider details upon contract execution. The RFP also addresses various operational requirements, such as dialysis medication procurement, biomedical waste disposal, staff training and clearance, and delivery of dialysis supplies. Pricing is to be provided using the rate sheet in Exhibit B-1, and the RFP notes that rates must be provided for all four institutions.

5 General Section 1.6 Key Action Dates
The agency states that answers to questions will be provided by Oct. 29th. However, the State's listed answer response deadline of Oct. 29th would not provide respondents with adequate time to incorporate answers that may have an impact on responses and then print, mail, and ship to meet the due date of Nov. 1. Would the agency be willing to provide the answers to questions on either a rolling basis or on an earlier date, or extend the submission deadline to accommodate inclusion of Q&A information?
Please refer to Addendum 2. The deadline for response to questions has been changed to October 22, 2024.
6 General Section 8.1 Proposal Requirements
27-28
Would the Agency accept a USB drive in place of a CD copy of the submitted proposal?
This requirement has been removed. Proposal's shall be sent through the mail/courier. Electronic submissions are not acceptable. The language in RFP Section 8.1 Proposal Requirements has been amended to reflect the following: E. The proposal must be completely sealed in an opaque container. Five (5) mandatory printed copies with one (1) copy marked as the “Master copy” must be received by the time and dates identified in Subsection 1.6 Key Action Dates. Additionally, an electronic CD copy of the proposal must be included with the proposal package.
