REQUEST FOR QUOTE FOR WELL SERVICES

PUMP AND MONITORING PORT INSTALLATIONS

Scope of Work

A. The 11 monitoring wells covered by this scope are described in Table 1. A map of the well locations is shown in Figure 1, and the driller’s reports for each well can be found at the end of this Attachment.

B. At each of the wells described in Table 1, the Contractor shall provide and install a submersible pump and motor, pitless adapter, a port for water quality sampling, and a 1” flush-jointed PVC pipe for water-level monitoring. Each pump that the Contractor installs shall pump water to the surface with a minimum flow rate of 5 gallons per minute using a Department generator (Generac XC8000E), which is rated for 10,000 starting watts and 8,000 running watts. Pump size and configuration (e.g., 2-wire or 3-wire) are to be determined based on the Contractor’s professional judgement. The depths to which the pumps and water-level monitoring pipes are to be set are listed in Table 1.

C. The Contractor shall provide and install a locking well monument at each well to protect the wellhead and associated monitoring equipment. The monument shall be constructed so that when the monument lid is opened it remains horizontal to the ground to provide a working surface, similar to the monument shown in Figure 2.

D. The Contractor will coordinate scheduling with the Department to complete the Scope of Work so that a Department employee will be on site during each installation.

E. While the Contractor is on site, a Department employee will pump the well using a Department generator (Generac XC80000E) to ensure the pump is operational and compatible with Department equipment. Each pump must be operational and compatible with Department equipment before the Department will approve payment.

F. The Scope of Work shall be completed by October 31, 2023. If the Contractor needs additional time to complete the Scope of Work, the Contractor must notify and seek written approval from the Contract Manager.
