About this file
This is a pricing matrix schedule for Request for Proposal No. 210000001912 issued by the State of Michigan Department of Health and Human Services Bureau of Grants & Purchasing for Families Together Building Solutions services. The RFP seeks proposals for the delivery of Families Together Building Solutions services over a three-year contract period, with contractors required to specify the proposed annual number of units to be provided for each fiscal year. Contractors must complete all sections of Schedule B, as removing or failing to respond to any questions will result in an incomplete proposal designation.
Pricing submissions must include a unit rate for services, proposed annual allocations for transportation (mileage reimbursement), and proposed annual allocations for specific assistance for each of the three fiscal years. The total annual contract value is calculated by multiplying the unit rate by the number of units, then adding the transportation and specific assistance amounts, with all proposals required to remain at or below the maximum award amount per contract year. For travel costs including mileage, meals, and lodging incurred in service delivery, contractors may bill the State of Michigan at the premium state rate or their usual employee reimbursement rate, whichever is less, with state travel rates available on the Michigan Department of Management and Budget website.
